はじめに
GMI（Global Medical Institute）は、ヒルズグレイスクリニック内に設立された教育機関で、美容医療の世界基準の向上を目標に、受講医師のニーズに応じた理論と実践の両面からの包括的なトレーニングを提供しています。座学でエネルギーベースデバイス（EBD、光や高周波などのエネルギーを用いる医療機器）の科学的原理や皮膚科学の基礎を学んだのち、ライブデモンストレーションなどの実践トレーニングを行う体系的なアプローチを採用しています。所在地は神奈川県横浜市のヒルズグレイスクリニック内です。
2024年は計5回のグローバルセミナーを開き、マレーシア、シンガポール、インドネシア、フィリピン、香港、中国、タイなど、各国・地域の医師を迎えました。ピコシュアプロやポテンツァ、サイノグローの理論と実践を中心に、シミ治療や毛穴・肌質改善への応用をお伝えしています。
この年の主な出来事です。
- 第4回セッション（中国編）では、新たにピコシュアプロを導入した中国全土の医師11名を対象に、ケーススタディ形式の実践的トレーニングが行われました。
- 第5回セッションでは、タイから参加した医師12名のリクエストに応じ、ポテンツァ単独の研修が実施されました。
- 2024年11月、Cynosure社よりプレステージクリニックとGlobal Training Facilityの2つの認定を同時に受けたことが発表され、同社から2つの認定を同時に受けた事例として紹介されました。
- 同時期に、臨床教育における功績を評価するClinical Education Recognition Awardと、臨床研究への貢献が認められたClinical Research Recognition Awardの2つの賞も受賞しました。
- 各セッションを通じて、プリセットモードに頼らずマニュアルモードで患者ごとに治療を組み立てる考え方が繰り返し共有されました。

2. セッションで扱った治療
この年のGMIで繰り返し扱ったのは、次の治療です。当院での料金・リスク・承認状況は、記事の後半「治療について」にまとめています。
ピコシュアプロ（PicoSure Pro）
PicoSure Proピコシュアプロ
ピコ秒（1兆分の1秒）単位でレーザーを照射する機器です。先代のピコシュアが自動設定を多く使う設計だったのに対し、ピコシュアプロは、レーザーの強さ・深さ・広がり・周囲への影響を、術者が自ら設定する必要があります。GMIでは、ポテンツァの「オートマ」と「マニュアル」にたとえて、ピコシュアが「オートマ」なら、ピコシュアプロは「マニュアル：レーシングモード」にあたるとお話ししました。
2024年は、第1回でサイノグローとあわせて扱い、第2回ではシミ治療へのピコシュアプロとピコシュアの応用をテーマにしました。第4回は、中国でピコシュアプロを導入した医師11名を対象に、理論講義のあとケーススタディを行い、「この色素斑は何か」「どこに存在しているのか」「なぜその色調になり、どのように破壊できるのか」「肌を傷つけずに後処理を行う方法は」という問いを、症例ごとに繰り返し検討しました。必要な回数や効果には個人差があります。
ポテンツァ（POTENZA）とサイノグロー
PicoSure Proピコシュアプロ
POTENZAポテンツァ講演当時の機種
ポテンツァは、極細の針を皮膚に入れながら高周波（RF）を流すマイクロニードルRFです。高周波には流れやすい状態（部位）と流れにくい状態があり、どの層に針を進め、どのような性質の高周波を使うかを決めることが出発点になります。目的に応じて、コラーゲン（肌のハリを支える成分）を焦がすのか、活性化するのかを調整する、繊細な作業だとお伝えしました。第1回では、ピコシュアプロとポテンツァを組み合わせる治療法「サイノグロー」も扱いました。
2024年は、第2回で毛穴や肌質改善への応用を、第3回で毛穴治療における針の角度とRFエネルギーの調整を扱い、第5回はタイの医師12名のリクエストに応じたポテンツァ単独の回になりました。プリセット（あらかじめ用意された設定）の「オートマ」モードでは一人ひとりの肌に合わせきれないため、テイラーメイドの設定ができる「マニュアル」モードの考え方を共有し、設定が適切でないと色素沈着や肌のへこみにつながるリスクにも触れました。現在、当院の公式案内でご紹介しているポテンツァは次世代機のポテンツァプライムで、国内未承認医療機器です。効果や経過には個人差があります。

テーマ ピコシュアプロ・ポテンツァ・サイノグローの理論と実践
ヒルズグレイスクリニック内に設立されたGMI（Global Medical Institute）の初回セッションとして、マレーシア、シンガポール、インドネシアの医師を招き、ピコシュアプロ（最新型ピコレーザー）、ポテンツァ（RFマイクロニードリング、高周波を使った針治療）、サイノグロー（両者を組み合わせた治療法）を効果的に使用するための科学的理論と実践的な技術を1日で学ぶ研修を行いました。
理論と実践を組み合わせた1日研修を通じて、参加医師が機器治療の科学的な理解を深める機会となりました。

テーマ シミ治療へのピコシュアプロ応用と、毛穴・肌質改善のためのポテンツァ応用
第1回から2週間後に開催された第2回セッションでは、インドネシア、フィリピン、マレーシアの医師を迎え、シミ治療に対するピコシュアプロとピコシュアの効果的な応用方法、毛穴や肌質改善のためのポテンツァの応用法をテーマに研修を行いました。
レーザーとニードルRFの違いを理解した上で治療を設計する重要性を、対話形式のセッションを通じて学ぶ機会となりました。

テーマ レーザー理論とニードルRFの組み合わせ、毛穴治療における針の角度・RFエネルギー調整
3回目となるこのセッションでは、香港、マレーシア、インドネシアから医師を迎え、レーザー理論とニードルRF（高周波を用いた針治療）の組み合わせ方、毛穴治療における針の角度やRFエネルギー調整の実践をテーマに研修を行いました。
プリセットモードに頼らず、患者ごとにマニュアルモードで治療を組み立てる考え方を共有するセッションとなりました。

上記はクリニック公式案内の料金の目安です（税込・2026年10月23日時点）。サイノグローは、ピコシュアプロとポテンツァプライムを同じ日に組み合わせる場合の料金です。ピコシュアプロで麻酔を行う場合は、麻酔代が別途かかります。診察・診断料は施術費用とは別に発生します（初めてご来院の方11,000円、前回のご来院から24か月以内の方1,100円、25か月以上の方11,000円）。料金や内容は変更される場合があります。
- 主な副作用・リスク
- PicoSure Proでは、皮膚の不快感、赤み、腫れ、点状出血、内出血、水疱、かさぶた、膿疱、色素沈着、色素脱失、熱傷、瘢痕、感染、アレルギー反応などが起こることがあり、シミの部分が一時的に濃く見えることがあります。ポテンツァプライムでは、照射時の痛み、一時的な赤み・むくみ・点状出血・かさぶた・内出血のほか、感染や色素沈着が起こることがあります。妊娠中・授乳中の方、ペースメーカー等の医療機器を体内に埋め込んでいる方は、照射治療を受けられない場合があります。複数の機器を組み合わせる治療の有効性・安全性は十分に確立されていません。適応、回数、ダウンタイムには個人差があります。
- 未承認医療機器に関する事項
- 【ポテンツァプライム／POTENZA PRIME・Jeisys Medical Inc.（韓国）】本治療に使用するポテンツァプライムは、日本国内において医薬品医療機器等法上の承認を得ていない未承認医療機器です。ポテンツァプライムは、製造元からの依頼にもとづき、当院で評価を目的として使用しています。旧機種のポテンツァは、国内販売元であるサイノシュアー・ルートロニック株式会社を通じて入手しています。同じシリーズの機器として、Jeisys Medical Japan株式会社の「ポテンツァBASIC」（承認番号 30500BZI00042000、一般的名称：治療用電気手術器）が、2023年12月に「審美性の改善のための皮膚のフラクショナルリサーフェシングを目的とした軟組織の凝固及び蒸散」を使用目的として承認されています。当院で使用しているポテンツァプライムは、この承認品ではありません。旧機種のポテンツァは、米国FDAの510(k)クリアランス（K192545ほか：皮膚科・一般外科の処置における電気凝固と止血）と、韓国MFDS（食品医薬品安全処）の承認を受けています。ポテンツァプライムとしての各国の承認状況は、現在確認中です。紅斑、腫脹、感染、色素沈着、熱傷などが報告されています。【PicoSure Pro】国内承認医療機器です（承認番号 30500BZX00079000／一般的名称：アレキサンドライトレーザ／使用目的又は効果：体表面の表在性及び深在性良性色素性病変の治療、又は外傷性並びに入墨による刺青の蒸散及び除去。ニキビ痕・毛穴・小ジワ・肌質を目的とする照射は適応外使用です）。

GMIの研修は、どのような形式で行いましたか？
午前に機器の科学的な原理や皮膚科学の基礎を学ぶ理論講義、午後にその理論を実際の施術で見るライブデモンストレーションという、1日がかりの構成から始めました。座学では、出た質問や意見を参加者全員やグループで議論し、知識の背景を掘り下げました。第4回では症例をもとにしたケーススタディを、第5回では「あなたならどのように治療するか」をテーマにした議論も行いました。皮膚科学・組織学・物理学・生物学の知識が前提になるため、研修の対象は医師に限っていました。
なぜ、プリセットモードではなくマニュアルモードを重視したのですか？
肌の厚みや状態は一人ひとり異なり、機器にあらかじめ用意された設定（プリセット）では、それぞれの肌に合わせきれないことがあるためです。第3回と第5回では、ポテンツァを例に、どこまで熱を届かせたいのか、どこを避けたいのかを考えて設定するマニュアルモードの考え方をお伝えしました。設定が適切でないと、熱が意図しない部分に作用し、色素沈着や肌のへこみにつながるリスクがあることにも触れました。効果や経過には個人差があります。
ピコシュアプロのセッションでは、何を伝えましたか？
ピコシュアプロは、レーザーの強さ・深さ・広がり・周囲への影響を術者が自ら設定する機器で、従来のレーザーと同じ感覚では扱えないことをお伝えしました。第4回では、中国でピコシュアプロを導入した医師11名と、色素斑の種類、存在する深さ、その色調になる理由と破壊の方法、肌を傷つけない後処理の方法を、ケーススタディで繰り返し検討しました。肌の状態・シミの性質・レーザーの特性をすべて考え合わせて治療を組み立てるのが、医師の役割だという考え方です。必要な回数や効果には個人差があります。
Global Training Facilityの認定とは何ですか？
2024年11月に、当院がCynosure社から受けたことを公表した認定です。当院の教育機関であるGMIが、世界各国の医師を対象に技術の教育を行う施設として選ばれました。GMIはヒルズグレイスクリニック内に当院が設立した教育機関で、この認定は当院が受けたものです。
GMIで扱った治療は、ヒルズグレイスクリニックで受けられますか？

In 2024, GMI (Global Medical Institute), based within Hills Grace Clinic, held a total of five global seminars, sharing the theory and practice of PicoSure Pro, Potenza and other devices with physicians from countries and regions including Malaysia, Singapore, Indonesia, the Philippines, Hong Kong, China and Thailand. The training is characterised by a systematic structure that combines classroom lectures on device and dermatological theory with live demonstrations of actual treatments. In November of that year, it was announced that the clinic had received Prestige Clinic and Global Training Facility accreditation from Cynosure, as well as two awards. The approach of designing treatment for each patient in manual mode, rather than relying on preset modes, was also shared consistently across the sessions.
Introduction
GMI (Global Medical Institute) is an educational institution established within Hills Grace Clinic. With the aim of raising global standards in aesthetic medicine, it provides comprehensive training covering both theory and practice, tailored to the needs of participating physicians. It takes a systematic approach: participants first learn the scientific principles of energy-based devices (EBDs, medical devices that use energy such as light or radiofrequency) and the fundamentals of dermatology in lectures, followed by practical training such as live demonstrations. It is located within Hills Grace Clinic in Yokohama, Kanagawa.
In 2024, we held a total of five global seminars, welcoming physicians from countries and regions including Malaysia, Singapore, Indonesia, the Philippines, Hong Kong, China and Thailand. Focusing on the theory and practice of PicoSure Pro, Potenza and CynoGlow®, we shared their application to the treatment of pigmentation and to pores and skin-quality improvement.
The main events of the year were as follows.
- In the 4th session (China edition), practical case-study-based training was held for 11 physicians from across China who had newly introduced PicoSure Pro.
- In the 5th session, training dedicated solely to Potenza was held at the request of 12 physicians who attended from Thailand.
- In November 2024, it was announced that the clinic had simultaneously received two accreditations from Cynosure, Prestige Clinic and Global Training Facility, and this was presented as a case of receiving two accreditations from the company at the same time.
- Around the same time, the clinic also received two awards: the Clinical Education Recognition Award, recognising achievements in clinical education, and the Clinical Research Recognition Award, recognising contributions to clinical research.
- Throughout the sessions, the approach of building each patient’s treatment in manual mode, rather than relying on preset modes, was shared repeatedly.
This article brings together, in a single piece, the reports on GMI sessions that I wrote for Grace Journal at the time.
The content of each session is based on the reports written at the time. Treatment results and progress vary between individuals, and many of the treatments covered are private, self-funded care (not covered by health insurance).

PicoSure Pro
PicoSure Pro
PicoSure Pro is a device that delivers laser energy in picosecond (one-trillionth of a second) units. Whereas its predecessor, PicoSure, was designed to make extensive use of automatic settings, PicoSure Pro requires the operator to set the laser’s intensity, depth, spread and effect on the surrounding area themselves. At GMI, I drew an analogy with Potenza’s “automatic” and “manual” modes: if PicoSure is the “automatic”, PicoSure Pro corresponds to “manual: racing mode”.
In 2024, PicoSure Pro was covered together with CynoGlow® in the 1st session, and the 2nd session focused on applying PicoSure Pro and PicoSure to the treatment of pigmentation. The 4th session was for 11 physicians who had introduced PicoSure Pro in China; after a theory lecture, we held case studies, repeatedly examining the following questions case by case: “What is this pigmented lesion?”, “Where is it located?”, “Why does it have this tone, and how can it be destroyed?” and “How can post-treatment care be carried out without damaging the skin?” The number of treatments needed and the results vary between individuals.

Potenza is a microneedle RF device that delivers radiofrequency (RF) energy while ultra-fine needles are inserted into the skin. RF flows easily in some conditions (areas) and less easily in others, so the starting point is deciding which layer to advance the needles to and what kind of RF to use. I explained that it is delicate work, adjusting whether to char or to activate collagen (a component that supports skin firmness) depending on the aim. The 1st session also covered CynoGlow®, a treatment method that combines PicoSure Pro and Potenza.
In 2024, the application of Potenza to pores and skin-quality improvement was covered in the 2nd session, and needle angle and RF energy adjustment in pore treatment in the 3rd; the 5th session was devoted solely to Potenza at the request of 12 physicians from Thailand. Because the preset (pre-programmed settings) “automatic” mode cannot fully adapt to each individual’s skin, we shared the approach of the “manual” mode, which allows tailor-made settings, and also touched on the risk that inappropriate settings can lead to hyperpigmentation and depressions in the skin. The Potenza currently featured in our clinic’s official information is the next-generation Potenza Prime, which is not approved as a medical device in Japan. Theme Theory and practice of PicoSure Pro, Potenza and CynoGlow®
As the first session of GMI (Global Medical Institute), established within Hills Grace Clinic, we invited physicians from Malaysia, Singapore and Indonesia to a one-day training course on the scientific theory and practical techniques for using PicoSure Pro (the latest picosecond laser), Potenza (RF microneedling, a needle treatment using radiofrequency) and CynoGlow® (a treatment method combining the two) effectively.
Through a one-day course combining theory and practice, the session gave participating physicians an opportunity to deepen their scientific understanding of device-based treatment.

Theme Applying PicoSure Pro to pigmentation treatment, and applying Potenza to pores and skin-quality improvement
The 2nd session, held two weeks after the 1st, welcomed physicians from Indonesia, the Philippines and Malaysia, with training on effective ways of applying PicoSure Pro and PicoSure to pigmentation treatment, and on applying Potenza to pores and skin-quality improvement.
Through an interactive session, it was an opportunity to learn the importance of designing treatment based on an understanding of the differences between lasers and needle RF.

Theme Combining laser theory with needle RF; adjusting needle angle and RF energy in pore treatment
In this third session, we welcomed physicians from Hong Kong, Malaysia and Indonesia, with training on how to combine laser theory with needle RF (a needle treatment using radiofrequency) and on the practice of adjusting needle angle and RF energy in pore treatment.
The session shared the approach of building each patient’s treatment in manual mode rather than relying on preset modes.

- With PicoSure Pro, skin discomfort, redness, swelling, petechiae, bruising, blisters, scabs, pustules, hyperpigmentation, hypopigmentation, burns, scarring, infection, allergic reactions and other effects may occur, and areas of pigmentation may temporarily look darker. With Potenza Prime, pain during treatment, temporary redness, oedema, petechiae, scabs and bruising, as well as infection and hyperpigmentation, may occur. People who are pregnant or breastfeeding, or who have an implanted medical device such as a pacemaker, may not be able to receive these device treatments. The efficacy and safety of treatments combining multiple devices have not been sufficiently established. Suitability, number of sessions and downtime vary between individuals.
- Unapproved medical devices
- [Potenza Prime / POTENZA PRIME, Jeisys Medical Inc. (South Korea)] Potenza Prime, used in this treatment, is an unapproved medical device that has not received approval in Japan under the PMD Act. Our clinic uses Potenza Prime for evaluation purposes at the request of the manufacturer. The earlier model, Potenza, is obtained through Cynosure Lutronic K.K., its distributor in Japan. As a device in the same series, Jeisys Medical Japan K.K.’s “Potenza BASIC” (approval number 30500BZI00042000; generic name: electrosurgical unit for treatment) was approved in December 2023 for the intended use of “coagulation and vaporisation of soft tissue for the purpose of fractional resurfacing of the skin to improve aesthetic appearance”. The Potenza Prime used at our clinic is not this approved product. The earlier model, Potenza, has received US FDA 510(k) clearance (K192545 and others: electrocoagulation and haemostasis in dermatological and general surgical procedures) and approval from Korea’s MFDS (Ministry of Food and Drug Safety). The approval status of Potenza Prime itself in each country is currently being confirmed. Erythema, swelling, infection, hyperpigmentation, burns and other effects have been reported. [PicoSure Pro] An approved medical device in Japan (approval number 30500BZX00079000 / generic name: alexandrite laser / intended use or effect: treatment of superficial and deep benign pigmented lesions of the body surface, or vaporisation and removal of traumatic tattoos and decorative tattoos. Treatment aimed at acne scars, pores, fine wrinkles or skin quality is off-label use).

What is GMI (Global Medical Institute)?
GMI (Global Medical Institute) is our clinic’s educational institute for physicians, established within Hills Grace Clinic in Azamino, Yokohama. With the aim of training physicians who can provide safe and effective medicine grounded in theory, it offers lectures on device theory and dermatology followed by practical training such as live demonstrations. In 2024, our clinic received certification from Cynosure as a Global Training Facility (a facility that provides training to physicians). GMI is an educational institute established by our clinic, and this certification was received by our clinic.
What sessions did GMI hold in 2024?
In 2024 we held Global Seminars 1–5. Session 1 in June (Malaysia, Singapore, Indonesia) covered PicoSure Pro, Potenza and CynoGlow®; Session 2 (Indonesia, the Philippines, Malaysia) covered PicoSure Pro for pigmentation treatment and Potenza for pores and skin-quality improvement; and Session 3 in September (Hong Kong, Malaysia, Indonesia) covered laser theory and how to combine lasers with needle RF. This was followed by Session 4, training on PicoSure Pro with 11 doctors from China, and Session 5 in November, training on Potenza alone with 12 doctors from Thailand.
What format did the GMI training take?
We began with a full-day format: theory lectures in the morning on the scientific principles of the devices and the fundamentals of dermatology, and live demonstrations in the afternoon showing that theory in actual treatments. In the lectures, questions and opinions that came up were discussed by all participants or in groups, to dig deeper into the background of the knowledge. Session 4 also included case studies based on actual cases, and Session 5 included discussions on the theme “How would you treat this?”. Because knowledge of dermatology, histology, physics and biology is a prerequisite, the training was limited to physicians.
Why did you emphasise manual mode rather than preset mode?
Because skin thickness and condition differ from person to person, and the settings prepared in advance in a device (presets) cannot always be fully matched to each individual’s skin. In Sessions 3 and 5, using Potenza as an example, we explained the manual-mode approach of setting parameters by considering how far the heat should reach and which areas should be avoided. We also noted that if the settings are not appropriate, the heat may act on unintended areas, with a risk of leading to pigmentation or depressions in the skin. Results and progress vary between individuals.
What did you convey in the PicoSure Pro sessions?
We explained that PicoSure Pro is a device with which the practitioner personally sets the laser’s intensity, depth, spread and effect on surrounding tissue, and that it cannot be handled with the same feel as conventional lasers. In Session 4, with 11 doctors who had introduced PicoSure Pro in China, we repeatedly examined through case studies the types of pigmented lesions, the depth at which they lie, why they take on their particular tone and how to break them down, and aftercare methods that do not damage the skin. The idea is that it is the physician’s role to build the treatment by considering the skin’s condition, the nature of the pigmentation and the characteristics of the laser all together. The number of sessions needed and the results vary between individuals.
What is the Global Training Facility certification?
It is a certification from Cynosure that our clinic announced in November 2024 it had received. GMI, our clinic’s educational institute, was selected as a facility that provides technical training for physicians from countries around the world. GMI is an educational institute established by our clinic within Hills Grace Clinic, and this certification was received by our clinic.
Can I receive the treatments covered at GMI at Hills Grace Clinic?
GMI is training for physicians; it is not something patients attend. PicoSure Pro, Potenza Prime (in 2024, the earlier model, Potenza) and CynoGlow®, which were covered in the sessions, are described on the respective treatment pages of the official website of Hills Grace Clinic in Azamino, Yokohama. All of them are private, self-funded care (not covered by health insurance), and Potenza Prime is not approved as a medical device in Japan. In closing
What I convey at society meetings and lectures is the same way of thinking I bring to each patient in the examination room. There are reasons behind device settings and the order of treatments, and by sharing those reasons with doctors overseas and discussing them again and again, our clinic’s treatments have also been updated. I hope this record can serve as one source of information for those considering treatment.

When I address concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when treatment is given at the same settings. What creates that difference is whether the skin is in a state where it can receive the heat and respond to treatment. That is why, while preparing the skin as needed so that it can respond, I propose the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper was selected as a Top Viewed and Top Cited Article of the journal. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
ASLMS specialist (American Society for Laser Medicine and Surgery) and Chair of the Board of Koyukai Medical Corporation. I am the only participant from Japan on Cynosure’s advisory board, have been involved with PicoSure Pro since its development stage, and have reported cases treated with the first unit. I have given invited lectures at international congresses such as ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also engaged in teaching and training doctors from overseas.
