私が開発した治療法であるCynoGlow（サイノグロー）の英語論文（industrial white paper）が発行され、商標登録も承認されました。
サイノグローは、肌質改善治療法です。シミの除去だけでなく、毛穴の改善、ニキビ痕の軽減、肌質の向上など、肌全体の美しさを向上させるために開発しました。
サイノグローでは、ピコ秒アレキサンドライトレーザー（PicoSure® pro/PicoSure®）とマイクロニードルRF機器（POTENZA®）を使用します。ピコ秒アレキサンドライトレーザーは、短いパルス幅で肌の色素斑やメラノソーム（色素細胞）をターゲットにして照射します。一方、マイクロニードルRF機器は微細なニードルを使い、RFエネルギーを皮膚の深部に送り込むことで熱を発生させます。
この治療法の特徴は、レーザーとニードルRFの弱点を相互補完しながら、お互いの強みを引き出すことです。これにより、肌のリジュビネーション（若返り）が促進され、色素斑の除去や真皮細胞外マトリクス（皮膚の組織構造）の再構築が行われます。その結果、肌のトーンが均一化され、毛穴が引き締まり、ニキビ痕が軽減され、肌質が改善されることが期待されます。

- 未承認医療機器に関する事項
- 【ポテンツァプライム／POTENZA PRIME・Jeisys Medical Inc.（韓国）】本治療に使用するポテンツァプライムは、日本国内において医薬品医療機器等法上の承認を得ていない未承認医療機器です。ポテンツァプライムは、製造元からの依頼にもとづき、当院で評価を目的として使用しています。症例で使用した旧機種のポテンツァは、国内販売元であるサイノシュアー・ルートロニック株式会社を通じて入手しています。同じシリーズの機器として、Jeisys Medical Japan株式会社の「ポテンツァBASIC」（承認番号 30500BZI00042000、一般的名称：治療用電気手術器）が、2023年12月に「審美性の改善のための皮膚のフラクショナルリサーフェシングを目的とした軟組織の凝固及び蒸散」を使用目的として承認されています。当院で使用しているポテンツァプライムは、この承認品ではありません。旧機種のポテンツァは、米国FDAの510(k)クリアランス（K192545ほか：皮膚科・一般外科の処置における電気凝固と止血）と、韓国MFDS（食品医薬品安全処）の承認を受けています。ポテンツァプライムとしての各国の承認状況は、現在確認中です。紅斑、腫脹、感染、色素沈着、熱傷などが報告されています。PicoSure Proは国内承認医療機器です（承認番号 30500BZX00079000／一般的名称：アレキサンドライトレーザ／使用目的又は効果：体表面の表在性及び深在性良性色素性病変の治療、又は外傷性並びに入墨による刺青の蒸散及び除去）。ニキビ・ニキビ痕・毛穴・小じわ・肌質・肝斑を目的とする照射は、承認された使用目的の範囲外の適応外使用です。治療内容は診察時に説明します。

The English paper (industrial white paper) on CynoGlow, the treatment I developed, has been published, and its trademark registration has also been approved.
CynoGlow® is a skin-quality improvement treatment. I developed it to enhance the beauty of the skin as a whole: not only removing dark spots, but also improving pores, reducing acne scars and improving skin quality.
CynoGlow® uses a picosecond alexandrite laser (PicoSure® pro/PicoSure®) and a microneedle RF device (POTENZA®). The picosecond alexandrite laser delivers treatment with short pulse widths, targeting pigmented lesions and melanosomes (pigment cells) in the skin. The microneedle RF device, on the other hand, uses fine needles to deliver RF energy deep into the skin, generating heat.
A feature of this treatment is that the laser and needle RF compensate for each other’s weaknesses while drawing out each other’s strengths. This promotes skin rejuvenation, with removal of pigmented lesions and remodelling of the dermal extracellular matrix (the structural tissue of the skin). As a result, a more even skin tone, tighter pores, reduced acne scars and improved skin quality can be expected.

- With PicoSure Pro, skin discomfort, redness, swelling, petechiae, bruising, blisters, crusting, pustules, hyperpigmentation, hypopigmentation, burns, scarring, infection, allergic reactions and other effects may occur. With Potenza Prime, redness, swelling, oedema, pain, petechiae, bruising, crusting, exudation of lymph fluid, infection, hyperpigmentation, hypopigmentation, burns, scarring and other effects may occur. The efficacy and safety of treatments combining multiple devices have not been fully established. Suitability, number of sessions and downtime vary between individuals.
- Unapproved medical devices
- [Potenza Prime / POTENZA PRIME, Jeisys Medical Inc. (South Korea)] The Potenza Prime used in this treatment is an unapproved medical device that has not received approval in Japan under the PMD Act. Our clinic uses Potenza Prime for evaluation purposes at the request of the manufacturer. The earlier Potenza model used in the cases was obtained through the domestic distributor, Cynosure Lutronic K.K. As a device in the same series, Jeisys Medical Japan K.K.’s “Potenza BASIC” (approval number 30500BZI00042000; generic name: electrosurgical unit for treatment) was approved in December 2023 for the intended use of “coagulation and vaporisation of soft tissue for the purpose of fractional resurfacing of the skin to improve aesthetic appearance”. The Potenza Prime used at our clinic is not this approved product. The earlier Potenza model has received US FDA 510(k) clearance (K192545 and others: electrocoagulation and haemostasis in dermatological and general surgical procedures) and approval from Korea’s MFDS (Ministry of Food and Drug Safety). The approval status of Potenza Prime itself in each country is currently being confirmed. Erythema, swelling, infection, hyperpigmentation, burns and other effects have been reported. PicoSure Pro is a medical device approved in Japan (approval number 30500BZX00079000 / generic name: alexandrite laser / intended use or effect: treatment of superficial and deep benign pigmented lesions of the body surface, or vaporisation and removal of traumatic tattoos and decorative tattoos). Treatment aimed at acne, acne scars, pores, fine wrinkles, skin quality or melasma is off-label use outside the scope of the approved intended use.
