Carl(台湾)と同じテーブルに。

Director Kentaro Oku | Record of a lecture given on 8 June 2019 (expanded and restructured on 20 October 2026)
Key points
In 2019, on Day 2 of the Cynosure Asia Pacific Forum 2019 held in Macau, I gave a lecture and live demonstration of the RF (radiofrequency) device TempSure Envi. In the morning symposium, I spoke about our clinical experience in Japan and our approach of building each treatment around the individual patient. During the afternoon demonstration, a physician from the advisory board asked to receive the treatment in person, and the session ran for more than three hours. Here I describe that day, together with the three conditions I look for in a radiofrequency device: “effectiveness, pain and safety”.

Introduction
After the closed meetings of the previous day came Day 2, the main day of the forum. What I had prepared for this day was a lecture on the radiofrequency device TempSure Envi. What criteria do I use to choose a radiofrequency device, and how do I use it? So began a day of showing exactly that in front of physicians from many countries.
1. Three conditions for a radiofrequency device
RF (radiofrequency) devices are made by many different companies, but I believed that few radiofrequency devices could deliver good results safely.
What we therefore look for in a radiofrequency device is that it combines the following three qualities.
- High power, so that results can be expected
- Little pain during treatment
- A design that takes safety into account, with a low risk of burns and similar problems

2. The morning symposium: reporting our clinical experience in Japan
The device I chose as meeting those conditions was TempSure Envi, which was already highly popular in the United States. At the time, our clinic had been the first in Asia to introduce this device, and on this day my role was to report on that clinical experience.
At the symposium, Dr. Robert Weiss (USA) first explained the mechanical features of TempSure Envi, and I then spoke about our clinical experience in Japan.
TempSure Envi has therapeutic logic control (a mechanism that controls the treatment temperature) for delivering the heat needed for treatment to collagen and to the ligaments involved in lifting (tissue that supports the skin). It is designed to keep the risk of side effects such as burns low by constantly monitoring the skin temperature. Even so, depending on the condition of the skin and how the treatment is delivered, the risk is not zero.
In the lecture, I presented specific treatment methods, treatment plans and the course of treatment. Afterwards I received many questions and comments, and the response exceeded my expectations.

3. The afternoon live demonstration: “Try it on me!”
In the afternoon live demonstration (a demonstration in which actual treatments are performed in front of the audience), board members each demonstrated one of Cynosure’s devices, including TempSure Envi, SculpSure, PicoSure, RevLite and Icon. Here too, I was in charge of TempSure Envi.
With so many devices, I had expected the audience to be spread out, but when I entered the room a crowd had already gathered. With Dr. Weiss and Dr. Christine watching closely, this demonstration was under even more demanding conditions than the morning. While treating the patient, I explained how to tailor the course of treatment to that person, why I use this level of energy, and how much heat is needed.
Just as things were going smoothly and I was beginning to relax, Dr. Christine said with a smile, “Try it on me!” To treat a physician who is a core member of the Advisory Board in front of a large audience: the weight of that responsibility was far greater than that of the morning presentation.
Having replied “Of course, please,” I had to do everything I could. I proceeded while explaining how to move the handpiece (the hand-held part of the device that delivers the treatment) and how to change the energy to make the treatment comfortable.
4. A full-face treatment, a three-hour session, and questions and answers
What was planned as a single area turned into a request for the whole face, and I carried out the full tailor-made treatment (treatment built around each individual) that I had presented in the lecture. When I received positive comments, I finally felt the weight lift from my shoulders.
In the end, the demonstration session lasted more than three hours, and by the time it finished I had almost no energy left. However, there was to be one more general question-and-answer session at the end. Summoning the last of my strength, I went up on stage, and in addition to more questions than I had expected, other Advisory Board members also put questions to me.
At the gala dinner, I sat at the same table as Dr. Weiss and Dr. Carl (Taiwan). Although nervous, I had an enjoyable time. In recognition of our clinic’s achievements, I was also presented with a crystal shield.
At the venue, I exchanged information with physicians from China and Australia and with Dr. Tsuchiya, who had come from Japan; it was a forum where I could not let my guard down until the very last moment.
5. What I learned over two days, and our practice in Azamino, Yokohama
Over these two days, I was able to absorb a great deal that is hard to learn in Japan, including the mechanisms and results of treatments that tend to produce results, and the development of treatment methods from new angles. I feel it was a time as fruitful as several years of study in Japan.
At what was then Azamino Hills Skin Clinic (now Hills Grace Clinic), whenever we performed radiofrequency treatment, we also placed importance on weighing the three factors of “effectiveness, pain and safety” and building how heat is delivered around each individual patient. That approach continues in our practice today.
Frequently asked questions
Is TempSure Envi treatment painful?
TempSure Envi is designed to deliver the heat needed for treatment while monitoring the skin temperature, and “little pain during treatment” is also one of the conditions we set when choosing a radiofrequency device. However, how the heat feels varies between individuals, and also depends on the area treated and the condition of the skin. In the Macau demonstration, too, I explained how to move the handpiece and adjust the energy so that the treatment can be received comfortably. For details, please see the TempSure Envi page on our official website.
Is there a risk of burns with radiofrequency (RF) treatment?
Because radiofrequency treatment uses heat, it is not free from risks such as burns. TempSure Envi is designed to deliver treatment while monitoring the skin temperature, but risks can arise depending on the condition of the skin and how the treatment is delivered. We place importance on choosing devices designed with safety in mind and on adjusting how heat is delivered to suit the condition of the skin. Risks and precautions are explained at the consultation before treatment.
What is tailor-made treatment?
It is an approach to treatment in which the treatment area, energy and the way heat is delivered are built around each individual patient’s condition. At the Macau forum, I presented this approach in my lecture and carried it out in full during the live demonstration. Even with the same device, the appropriate treatment differs depending on the condition of the skin. How results appear varies between individuals.
In closing
The two days in Macau were a succession of meetings, lectures, demonstrations and question-and-answer sessions. The meetings of the previous day are described in the Day 1 article, and the forum held in Indonesia in the winter of the same year in a separate article. Once again, I would like to express my heartfelt thanks to the many people who gave me this opportunity.
