はじめに
同じレーザーを、科学の側と臨床の側から語る。タイでのこのイベントは、そうした役割分担で一つのテーマを掘り下げる場になりました。
1. Polnikorn教授とともに
Polnikorn教授は、「レーザートーニング」の考案者として知られるレーザーの専門家です。私たちはともにCynosure社のMedical Advisory Boardの一員として、医療機器の開発・改良に携わっています。
さらに、北アジアでは当院が、南アジアではPolnikorn教授の施設が、Research & Training Centerに認定されています。
2. チュラロンコン大学での「PICOSURE focus on versatile revolution」
会場は、タイの名門チュラロンコン大学です。レーザーイベント「PICOSURE focus on versatile revolution」には、各国から多くの医師が集まりました。
Polnikorn教授は科学的な観点から、ピコシュア・ピコシュアプロの重要性について講演されました。一方、私は臨床的な側面から、ピコシュア・ピコシュアプロとマイクロニードルRFのポテンツァを組み合わせた治療法、サイノグローについてお話ししました。
3. 臨床の視点から見たサイノグロー
サイノグローは、ピコ秒レーザーとマイクロニードルRFを組み合わせ、肌の若返りを促すことを目指して組み立てた治療です。講演では、色素への働きかけと、真皮の細胞外マトリクス（コラーゲンなど肌の土台となる成分）の再構築という二つの面から、その考え方を紹介しました。
サイノグローで使うピコシュアプロは国内承認の医療機器ですが、ニキビ・ニキビ痕・毛穴・肌質を目的とする照射は承認された使用目的の範囲外（適応外使用）です。現在使用しているポテンツァプライムは国内未承認医療機器です。料金・リスク・承認状況は、この記事の「治療について」に記載しています。効果や経過には個人差があります。
4. 質疑応答から得たもの
各国から集まった医師と、活発に質疑応答を行いました。参加者の多様な背景と専門知識に触れることで、私自身の知識を補い、新しい視点を得ることができました。

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

上記はクリニック公式案内の料金の目安です（税込・2026年10月20日時点）。サイノグローは、ポテンツァプライムとピコシュアプロを同じ日に組み合わせる場合の料金です。講演当時の記事に記載していた料金は当時のもので、現在の料金とは異なります。ピコシュアプロの表面麻酔代は別途かかります。診察・診断料は施術費用とは別に発生します（初めてご来院の方11,000円、前回のご来院から24か月以内の方1,100円、25か月以上の方11,000円）。実際の費用、照射範囲、回数は診察時にご確認ください。料金や内容は変更される場合があります。
- 主な副作用・リスク
- PICOSURE proでは、皮膚の不快感、赤み、腫れ、点状出血、内出血、水疱、かさぶた、膿疱、色素沈着、色素脱失、熱傷、瘢痕、感染、アレルギー反応などが起こることがあり、シミの部分が一時的に濃く見えることがあります。ポテンツァプライムでは、赤み、腫れ、むくみ、痛み、点状出血、内出血、かさぶた、リンパ液の浸出、感染、色素沈着、色素脱失、熱傷、瘢痕などが起こることがあります。複数の機器を組み合わせる治療の有効性・安全性は、十分に確立されていません。多くは一過性ですが、症状や経過、効果、回数、ダウンタイムには個人差があります。
- 未承認医療機器に関する事項
- 【ポテンツァプライム／POTENZA PRIME・Jeisys Medical Inc.（韓国）】承認状況：本治療に使用するポテンツァプライムは、日本国内において医薬品医療機器等法上の承認を得ていない未承認医療機器です。入手経路：ポテンツァプライムは、製造元からの依頼にもとづき、当院で評価を目的として使用しています。講演の症例で使用した旧機種のポテンツァは、国内販売元であるサイノシュアー・ルートロニック株式会社を通じて入手しています。国内の承認医療機器の有無：同じシリーズの機器として、Jeisys Medical Japan株式会社の「ポテンツァBASIC」（承認番号 30500BZI00042000、一般的名称：治療用電気手術器）が、2023年12月に「審美性の改善のための皮膚のフラクショナルリサーフェシングを目的とした軟組織の凝固及び蒸散」を使用目的として承認されています。当院で使用しているポテンツァプライムは、この承認品ではありません。諸外国における安全性等：旧機種のポテンツァは、米国FDAの510(k)クリアランス（K192545ほか：皮膚科・一般外科の処置における電気凝固と止血）と、韓国MFDS（食品医薬品安全処）の承認を受けています。ポテンツァプライムとしての各国の承認状況は、現在確認中です。紅斑、腫脹、感染、色素沈着、熱傷などが報告されています。【PICOSURE pro】国内承認医療機器です（承認番号 30500BZX00079000／一般的名称：アレキサンドライトレーザ／使用目的又は効果：体表面の表在性及び深在性良性色素性病変の治療、又は外傷性並びに入墨による刺青の蒸散及び除去）。ニキビ・ニキビ痕・毛穴・小じわ・肌質・肝斑を目的とする照射は、承認された使用目的の範囲外の適応外使用です。

Key points
In May 2023, at the laser event “PICOSURE focus on versatile revolution” held at Chulalongkorn University in Thailand, I gave a lecture together with Professor Polnikorn, known as the originator of “laser toning” (a treatment method in which a low-power laser is applied repeatedly). Professor Polnikorn spoke on the significance of PicoSure and PicoSure Pro from a scientific perspective, and I spoke from a clinical perspective on CynoGlow®, which incorporates Potenza. We are both members of Cynosure’s Medical Advisory Board.

1. With Professor Polnikorn
Professor Polnikorn is a laser specialist known as the originator of “laser toning”. As members of Cynosure’s Medical Advisory Board, we are both involved in the development and improvement of medical devices.
In addition, our clinic is accredited as a Research & Training Center for North Asia, and Professor Polnikorn’s facility as one for South Asia.
2. “PICOSURE focus on versatile revolution” at Chulalongkorn University
The venue was Chulalongkorn University, a prestigious university in Thailand. The laser event “PICOSURE focus on versatile revolution” brought together many physicians from various countries.
Professor Polnikorn lectured on the importance of PicoSure and PicoSure Pro from a scientific perspective. I, for my part, spoke from the clinical side about CynoGlow®, a treatment method that combines PicoSure and PicoSure Pro with the microneedle RF device Potenza.
3. CynoGlow® from a clinical perspective
CynoGlow® is a treatment designed with the aim of promoting skin rejuvenation by combining a picosecond laser with microneedle RF. In the lecture, I introduced the thinking behind it from two angles: acting on pigment, and rebuilding the extracellular matrix of the dermis (the components, such as collagen, that form the foundation of the skin).
PicoSure Pro, used in CynoGlow®, is a medical device approved in Japan, but treatment with it for acne, acne scars, pores and skin quality falls outside its approved intended use (off-label use). Potenza Prime, which we currently use, is not approved as a medical device in Japan. Fees, risks and approval status are listed in the “About the treatment” section of this article. Results and progress vary between individuals.
4. What I gained from the Q&A
I had a lively question-and-answer session with physicians from various countries. Encountering the participants’ diverse backgrounds and expertise allowed me to supplement my own knowledge and gain new perspectives.

What is the thinking behind CynoGlow®?
CynoGlow® combines the picosecond laser PicoSure Pro with the microneedle RF device Potenza, aiming to rejuvenate the skin from two sides: acting on pigment and rebuilding the components that form the foundation of the skin. Potenza Prime, which we currently use, is not approved as a medical device in Japan, and the treatment is private, self-funded care.
What are the approval status and fees of the treatments presented in the lecture?
All of them are private, self-funded care (not covered by health insurance). Whether a treatment suits you, the number of sessions, the fees and possible side effects will be explained after we have examined your skin at a consultation. When addressing concerns that appear on the skin, such as pigmentation, melasma or pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms have arisen.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when the same settings are used. What creates that difference is whether the skin is in a state to receive heat and respond to treatment. That is why I propose the appropriate treatments in the appropriate order, while preparing the skin, as needed, so that it is able to respond.
I made this idea the starting point of treatment design, systematised it as the “Thermal Thread Technique®”, and published it in 2024 in the journal “Lasers in Surgery and Medicine”. The paper has been selected as a Top Viewed and Top Cited Article in that journal. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
Specialist of the American Society for Laser Medicine and Surgery (ASLMS) and Chair of the Board of Koyukai Medical Corporation. As the only participant from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have reported cases using the first unit. I have given invited lectures at international congresses including ASLMS, the world’s largest society for laser medicine, IMCAS ASIA and KOREA DERMA, and lead the international physician education programme “GMI”. I am also engaged in teaching and training physicians overseas.
Even for those who have not had the results they hoped for from previous treatments, I assess why that skin is showing those symptoms and propose treatment starting from the order in which it is needed.
