2022年、アメリカ・サンディエゴのSan Diego Convention Centerで開かれたASLMS（アメリカレーザー医学会）の学術集会で、私の4つの演題が採択されました。テーマは、ピコシュアとシスペラの併用、ピコシュアプロとポテンツァによる肌質改善、ルクセンブルクのDr. Dierikxとの共著によるテンプシュアの治療法、スティムシュアとスカルプシュアによるボディメイクです。4演題のうち3つは、複数の治療を併用するコンビネーション治療の発表でした。いずれも効果の出方には個人差があります。
はじめに
2022年のASLMSは、私にとって4つの演題が採択された学会でした。そのうち3つは、2つの治療を組み合わせるコンビネーション治療の発表です。サンディエゴの会場でお話しした内容を、演題ごとに振り返ります。
1. ASLMS2022 ― 1月のLSM24から始まった1年
ASLMS（アメリカレーザー医学会）の学術集会は、2022年、アメリカ・カリフォルニア州のSan Diego Convention Centerで開かれました。毎年、新しい治療機器やアプローチの方法など、さまざまな視点から学びを得ることができる、レーザー機器の分野の大きな学会です。
私はこれまでもASLMSで発表を重ねてきましたが、2022年は、1月に行われたLSM24のパネリストに選ばれたことから始まりました。LSM24は、ASLMSの学術雑誌の編集者が各国から代表者を選び、レーザーについて自身の知見や研究を討論し合うオンラインイベントです。
そして学術集会では、次の4つの演題が採択されました。
- ピコシュア（PicoSure）とシスペラのコンビネーション治療
- ピコシュアプロ（PICOSURE pro）とポテンツァ（POTENZA）による肌質改善治療
- テンプシュア（TempSure）による治療法（Dr. Dierikxとの共著）
- スティムシュアとスカルプシュアによるボディメイキング
2. 演題1・2 ― ピコシュア×シスペラ、ピコシュアプロ×ポテンツァ
一つ目は、ピコシュアとシスペラのコンビネーション治療です。シスペラは、システアミンを配合した美白クリームです。レーザー治療の弱点のひとつに、炎症のあとに起こる色素沈着（炎症後色素沈着）があります。シスペラがこれを抑えることで、治療効率の向上が期待できることを示しました。
二つ目は、ピコシュアプロとポテンツァによる肌質改善治療です。ポテンツァは、針を使って高周波（RF）を届けるニードルRFの機器です。この内容は、少し前にモナコで開かれたAMWCでも講演していました。
レーザーとニードルRF、それぞれの弱点を補い合いつつ、お互いの強みを引き出せるのが、この治療の特長です。この治療は、その後オーストラリアのCOSDOCやタイ・バンコクのIMCAS Asiaでも、サイノグロー（CynoGlow）という名前でお話ししています。
3. 演題3 ― Dr. Dierikxと共著したテンプシュアの治療法
三つ目は、Cynosure社のAdvisory board memberとしてともに仕事をしていた、ルクセンブルクのDr. Dierikxとの共著演題です。
テーマは、高性能RF（高周波）機器テンプシュアによるオリジナルの治療法です。フェイスラインなどのたるみの改善に効果が期待できることを示しました。効果の出方には個人差があります。
テンプシュアは、当院がアジア太平洋地域での導入施設に選ばれた機器のひとつで、治療効果の引き出し方や使い方のコツについて、講演や技術指導も行ってきました。
4. 演題4 ― スティムシュアとスカルプシュアで考えるボディメイク
四つ目は、当時、当院がアジアで1号機を導入していた筋増強機器スティムシュア（StimSure）と、レーザーによる脂肪溶解を行うスカルプシュア（SculpSure）を組み合わせた、ボディコントゥアリング（ボディメイキング）についてです。
異なるエネルギーをブレンドすることで得られる効果と、3次元での評価方法を示しました。脂肪を減らしながら、体幹やインナーマッスルを鍛えるという考え方です。効果の出方には個人差があります。
このボディメイクの方法は、その後のJSAS2022（日本美容外科学会）でも、脂肪だけでなく筋肉と皮膚も同時にケアする「デュアルシュア」として、部分痩身の講演でお話ししました。
5. 横浜あざみ野の診療へ ― 肌質に合わせて組み合わせる
肌のコンビネーション治療については、JSAS2022でも、ピコシュアプロとポテンツァによる美肌治療、ピコシュアプロとシスペラによる美白治療という、当院の2つの治療としてお話ししています。どちらも、その方の肌質に合わせてプログラムを構築する治療です。横浜あざみ野の当院でも、レーザーとほかの治療の組み合わせは、一人ひとりの肌質に合わせたプログラムとして組み立てていました。
また、同じ年の日本美容皮膚科学会では、シスペラの新しいラインナップについてもお話ししました。効果や副反応の出方には個人差があり、適応は診察で判断します。

レーザーとニードルRFを併用し、それぞれの得意な部分を生かすことで、肌質の改善が期待できる治療です。のちにサイノグロー（CynoGlow）として講演したこの治療は、お肌に不要な色素が蓄積した部分にピコシュアプロを、ニキビや小じわといった真皮コラーゲンに関わる部分にポテンツァを照射する考え方です。効果の出方には個人差があり、適した治療は肌の状態によって異なります。

Key points
In 2022, four of my presentations were accepted at the annual meeting of ASLMS (American Society for Laser Medicine and Surgery), held at the San Diego Convention Center in San Diego, USA. The topics were the combination of PicoSure and Cyspera; skin-quality improvement with PicoSure Pro and Potenza; a TempSure Envi treatment method co-authored with Dr. Dierikx of Luxembourg; and body contouring with StimSure and SculpSure. Three of the four were presentations on combination treatments that use several treatments together. In every case, results vary between individuals.
Introduction
At ASLMS in 2022, four of my presentations were accepted. Three of them were on combination treatments that bring two treatments together. Here I look back, presentation by presentation, at what I spoke about in San Diego.
1. ASLMS 2022: a year that began with LSM24 in January
In 2022, the annual meeting of ASLMS (American Society for Laser Medicine and Surgery) was held at the San Diego Convention Center in California, USA. It is a major society in the field of laser devices, where every year one can learn from many perspectives, including new treatment devices and new approaches.
I had presented at ASLMS before, but 2022 began with my selection as a panellist for LSM24, held in January. LSM24 is an online event in which the editors of the ASLMS scientific journal select representatives from various countries to discuss their own findings and research on lasers.
At the annual meeting, the following four presentations were then accepted.
- Combination treatment with PicoSure and Cyspera
- Skin-quality improvement treatment with PicoSure Pro and Potenza
- A treatment method using TempSure Envi (co-authored with Dr. Dierikx)
- Body contouring with StimSure and SculpSure
2. Presentations 1 and 2: PicoSure × Cyspera, PicoSure Pro × Potenza
The first was combination treatment with PicoSure and Cyspera. Cyspera is a brightening cream containing cysteamine. One of the weaknesses of laser treatment is pigmentation that occurs after inflammation (post-inflammatory hyperpigmentation). I showed that, by Cyspera suppressing this, an improvement in treatment efficiency can be expected.
The second was skin-quality improvement treatment with PicoSure Pro and Potenza. Potenza is a needle RF device that uses needles to deliver radiofrequency (RF) energy. I had also lectured on this topic a little earlier at AMWC in Monaco.
The defining feature of this treatment is that the laser and the needle RF make up for each other’s weaknesses while drawing out each other’s strengths. I later also spoke about this treatment under the name CynoGlow® at COSDOC in Australia and at IMCAS Asia in Bangkok, Thailand.
3. Presentation 3: a TempSure Envi treatment method co-authored with Dr. Dierikx
The third was a presentation co-authored with Dr. Dierikx of Luxembourg, with whom I worked as a fellow Advisory board member of Cynosure.
The theme was an original treatment method using TempSure Envi, a high-performance RF (radiofrequency) device. We showed that it can be expected to help improve sagging along the jawline and elsewhere. Results vary between individuals.
TempSure Envi is one of the devices for which our clinic was selected as an introducing site in the Asia-Pacific region, and I have also given lectures and technical training on how to draw out its treatment effects and practical tips for its use.
4. Presentation 4: body contouring with StimSure and SculpSure
The fourth was on body contouring (body making) combining StimSure, a muscle-strengthening device of which our clinic had at the time installed the first unit in Asia, with SculpSure, which performs laser fat reduction.
I showed the effects obtained by blending different types of energy, and a method of three-dimensional evaluation. The idea is to reduce fat while strengthening the core and the inner muscles. Results vary between individuals.
I later also spoke about this body-contouring method in a lecture on localised slimming at JSAS 2022 (Japan Society of Aesthetic Plastic Surgery), as “DualSure”, which cares for muscle and skin at the same time as fat.
5. Into practice in Azamino, Yokohama: combinations tailored to skin type
On combination treatments for the skin, I also spoke at JSAS 2022 about two of our clinic’s treatments: a skin-beautifying treatment with PicoSure Pro and Potenza, and a skin-brightening treatment with PicoSure Pro and Cyspera. Both are treatments in which a programme is built to suit each person’s skin type. At our clinic in Azamino, Yokohama, too, combinations of laser and other treatments were put together as programmes tailored to each individual’s skin type.
In the same year, at the Japanese Society of Aesthetic Dermatology, I also spoke about the new Cyspera line-up. Effects and side reactions vary between individuals, and suitability is determined at an examination.

What can be expected from combining PicoSure Pro and Potenza?
It is a treatment that uses a laser and needle RF together, making use of the strengths of each, and from which an improvement in skin quality can be expected. The idea of this treatment, which I later lectured on as CynoGlow®, is to apply PicoSure Pro to areas where unwanted pigment has accumulated in the skin, and Potenza to concerns involving dermal collagen, such as acne and fine wrinkles. Results vary between individuals, and the suitable treatment depends on the condition of the skin.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when the treatment is delivered with the same settings. What creates that difference is whether the skin is in a state to receive the heat and respond to treatment. That is why, while preparing the skin so that it can respond where necessary, I propose the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper has been selected as a Top Viewed and Top Cited Article in that journal. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
ASLMS specialist (American Society for Laser Medicine and Surgery) and Chair of the Board of Koyukai Medical Corporation. As the only participant from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have also reported cases treated with the first unit. I have given invited lectures at international congresses including ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead “GMI”, an international physician education programme. I am also engaged in teaching and training doctors overseas.
Even for those who have not seen the results they hoped for from previous treatments, I assess why those symptoms are occurring in that particular skin and propose treatment starting from the order in which it is needed.
