はじめに
学会の公式プログラムとは別に、iSWAMの期間中にはプライベートセミナーも開かれました。
1. インドネシアのエキスパートKOLとのセミナー
iSWAMの期間中、インドネシアのエキスパートKOLに向けたプライベートセミナーが開かれました。Dr. Henryをはじめ、インドネシアの主要なKOLの医師たちが集まり、話題の中心はやはりピコシュアプロでした。
2. 性能を引き出し、副作用を抑えるために
どうすれば機器の性能を十分に引き出しながら、副作用を減らすことができるか。これは医師の腕の見せどころであり、同時に、一人ひとりの肌との対話が必要な、とても難しいポイントでもあります。
機器の性能を引き出すことと、副作用を減らすことを両立させるには、一人ひとりの肌の状態に向き合うことが欠かせません。
3. 私がピコシュアプロを使う理由
私がピコシュアプロを治療の中心に置いているのは、研究データにもとづいて照射を組み立てられるからです。そのことは、直前のAMWC Japan 2023でもお話ししました。
当院の治療では、シミの原因となるメラニンを取り除くだけでなく、肌そのものの再生を同時に目指します。そのため、シミだけでなく、ニキビ痕の色素沈着などにも用いています。ニキビ痕を目的とする照射は、ピコシュアプロの承認範囲外（適応外使用）です。効果には個人差があります。
よくある質問
レーザー治療で、副作用を抑えるために大切なことは何ですか？
一人ひとりの肌の状態に向き合い、それに合わせて照射を組み立てることだと考えています。副作用の出方には個人差があり、気になる変化があれば受診時にお知らせください。
ピコシュアプロは、ニキビ痕の色素沈着にも使えますか？
当院では、シミだけでなくニキビ痕の色素沈着などにも用いています。ただし、ピコシュアプロの国内で承認された使用目的は良性の色素性病変の治療と刺青の除去で、ニキビ痕を目的とする照射は適応外使用です。合うかどうかは診察で判断します。
iSWAMとは、どんな学会ですか？
インドネシアで開かれる国際学会です。私は2023年に2年連続で講師として招かれ、会期中には現地のエキスパートKOLに向けたピコシュアプロのプライベートセミナーも担当しました。

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

Key points
In November–December 2023, during iSWAM, an international congress in Indonesia, a private seminar was held for local expert KOLs (physicians in leading positions in their countries), and I spoke about PicoSure Pro. The theme was how to draw out its performance while reducing side effects. This is where a doctor’s skill is truly tested, and it is also a difficult point that requires a dialogue with each individual’s skin.
Introduction
Separately from the official congress programme, private seminars were also held during iSWAM.
1. Seminar with expert KOLs in Indonesia
During iSWAM, a private seminar was held for expert KOLs in Indonesia. Leading Indonesian KOL physicians, including Dr. Henry, gathered, and the main topic of discussion was, as expected, PicoSure Pro.
2. Drawing out performance while keeping side effects in check
How can we draw out the full performance of a device while reducing side effects? This is where a doctor’s skill is truly tested, and at the same time it is a very difficult point that requires a dialogue with each individual’s skin.
To achieve both, drawing out a device’s performance and reducing side effects, it is essential to attend to the condition of each individual’s skin.
3. Why I use PicoSure Pro
I place PicoSure Pro at the centre of my treatment because it allows me to plan laser treatment based on research data. I also spoke about this at AMWC Japan 2023, held just before.
In our clinic’s treatment, we aim not only to remove the melanin that causes pigmentation but also, at the same time, to regenerate the skin itself. For this reason, we use PicoSure Pro not only for pigmentation but also for conditions such as pigmentation in acne scars. Treatment aimed at acne scars is outside the approved scope of PicoSure Pro (off-label use). Results vary between individuals.
Frequently asked questions
In laser treatment, what is important for keeping side effects in check?
I believe it is attending to the condition of each individual’s skin and planning the laser treatment accordingly. How side effects appear varies between individuals; if you notice any change that concerns you, please let us know at your visit.
Can PicoSure Pro also be used for pigmentation in acne scars?
At our clinic, we use PicoSure Pro not only for pigmentation but also for conditions such as pigmentation in acne scars. However, the approved intended use of PicoSure Pro in Japan is the treatment of benign pigmented lesions and tattoo removal, and treatment aimed at acne scars is off-label use. Whether it is suitable for you is assessed at an examination.
What kind of congress is iSWAM?
It is an international congress held in Indonesia. In 2023 I was invited as a speaker for the second year in a row, and during the congress I also led a private PicoSure Pro seminar for local expert KOLs.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when the same settings are used. What creates that difference is whether the skin is in a state to receive the heat and respond to the treatment. That is why, where necessary, I prepare the skin so that it can respond, while proposing the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper has been selected as a Top Viewed and Top Cited Article in the journal. I have also published the radiofrequency technique “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in “Journal of Cosmetic Dermatology”.
ASLMS (American Society for Laser Medicine and Surgery) specialist and Chair of the Board of Koyukai Medical Corporation. As the only member from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have also reported cases treated with the first unit. I have given invited lectures at international congresses such as ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also engaged in teaching and training physicians overseas.
For those who have not had the results they hoped for from previous treatments, too, I assess why those symptoms are occurring in that skin and propose treatment starting from the order in which it is needed.
