はじめに
学会での講演とは別に、インドネシア滞在中にはもう一つの役割がありました。現地のクリニックで、実際の機器と患者様を前にしたトレーニングです。
1. 理論だけでは、機器は使いこなせない
レーザー機器は、理論が理解できても、それを使いこなすためにはさまざまな実践的なトレーニングが必要です。今回は、3つの施設から依頼を受けて、プライベートテクニカルトレーニングを行いました。
2. bmderma ― 難治性の色素斑とニキビ痕
最初の訪問先は、多くの企業のKOLを務めるDr. Deasyのクリニック、bmdermaです。難治性の色素斑やニキビ痕に対してどのように機器を使うのか、そしてほかの機器と組み合わせるときの注意点やコツについてお話ししました。
3. BLINK BEAUTY CLINIC ― 肌の診断から照射まで
続いて訪ねたのは、Dr. RaynaldのBLINK BEAUTY CLINICです。チーフドクターのDr. Raynaldたちに、レーザー治療に適した肌の診断方法をレクチャーしたうえで、症状に合わせたピコシュアプロの照射方法を、実際の患者様の症状に合わせて説明しました。
4. D’JANTARI dermatology ― 深く大きなシミに向き合う
最後に訪ねたのは、D’JANTARI dermatologyです。院長のDr. Ratnaたちと、長期間の紫外線によってできた深く大きなシミを、いかに安全かつ効果的に治療するかについて、肌の見極め方からお話ししました。

深く大きなシミは、どう見極めますか？
長期間の紫外線でできた深く大きなシミは、まず肌の状態を見極めるところから治療を考えます。シミの種類や深さ、肌の反応の出やすさによって、適した照射は変わります。どこまで改善が期待できるか、回数の見込みはどのくらいかは、診察でご説明します。

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

In November–December 2023, while staying in Indonesia, I was asked by three facilities to give private technical training on laser devices. Even when the theory behind a laser device is understood, practical training is essential to use it well. At each facility I covered, respectively, how to use the devices for refractory pigmented lesions and acne scars, how to diagnose skin suited to laser treatment and how to deliver the treatment, and how to assess the skin in order to treat deep, large dark spots safely.
Introduction
Apart from lecturing at the society meeting, I had one more role during my stay in Indonesia: training at local clinics, working with actual devices and patients.
1. Theory alone is not enough to master a device
Even when the theory behind a laser device is understood, a variety of practical training is needed to use it well. On this occasion, I was asked by three facilities to give private technical training.
2. bmderma: refractory pigmented lesions and acne scars
The first facility I visited was bmderma, the clinic of Dr. Deasy, who serves as a KOL for many companies. I talked about how to use the devices for refractory pigmented lesions and acne scars, and about the precautions and tips for combining them with other devices.
3. BLINK BEAUTY CLINIC: from skin diagnosis to treatment
Next I visited Dr. Raynald’s BLINK BEAUTY CLINIC. After giving the chief doctor, Dr. Raynald, and his colleagues a lecture on how to diagnose skin suited to laser treatment, I explained how to deliver PicoSure Pro treatment for each condition, tailored to the conditions of actual patients.
4. D’JANTARI dermatology: addressing deep, large dark spots
The last facility I visited was D’JANTARI dermatology. With the director, Dr. Ratna, and her colleagues, I discussed how to treat deep, large dark spots caused by long-term UV exposure as safely and effectively as possible, starting from how to assess the skin.

What does technical training involve?
It is hands-on teaching, working with actual devices and patients, that covers everything from how to diagnose the skin to how to deliver treatment. In December 2023, I was asked by three facilities in Indonesia to give private technical training on laser devices.
Why is skin diagnosis important before laser treatment?
Because even with the same device, the appropriate treatment method changes depending on the condition of the skin. By assessing the depth and type of the dark spots and how readily the skin reacts, we plan treatment that balances effectiveness and safety. The course after treatment varies between individuals.
How are deep, large dark spots assessed?
For deep, large dark spots caused by long-term UV exposure, we plan treatment by first assessing the condition of the skin. The appropriate treatment changes depending on the type and depth of the dark spots and how readily the skin reacts. How much improvement can be expected and roughly how many sessions may be needed are explained at the consultation.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when treatment is delivered at the same settings. What creates that difference is whether the skin is in a state to receive heat and respond to treatment. That is why, where necessary, I prepare the skin so that it can respond, while proposing the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper has been selected as a Top Viewed and Top Cited Article of the journal. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
Specialist of the American Society for Laser Medicine and Surgery (ASLMS) and Chair of the Board of Koyukai Medical Corporation. As the only member from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have reported cases using the first unit. I have given invited lectures at international congresses such as ASLMS, the world’s largest society of laser medicine, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also engaged in teaching and training doctors overseas.
Even for those who have not obtained the results they hoped for from previous treatments, I assess why those symptoms are occurring in that skin and propose treatment starting from the order in which it is needed.
