はじめに
レーザーでシミを治療しようとするとき、その結果を左右する大切なポイントがあります。2022年の美容外科学会総会で、私はそのひとつ、「レーザーが皮膚のどこまで届くか」に焦点を当てて、ピコシュアプロについてお話ししました。
1. 札幌での第45回美容外科学会総会
この年の美容外科学会総会（JSAPS2022）は第45回で、会場は札幌でした。講演のテーマは、ピコレーザー「ピコシュアプロ」です。
ピコシュアプロは、前身のピコシュアに大幅な改良を加えたピコレーザーです。私はCynosure社のAdvisory board memberとして開発に関わり、当時、当院はその世界における初号機を保有していました。
2. 出力を上げても、届く深さは変わらなかった
深いシミをしっかり取りたい場合、従来のピコシュア（PicoSure）では、出力を上げても深達度（レーザーが届く深さ）は変わりませんでした。
当院で使っていたピコシュアも優秀なレーザーでした。講演では、この届く深さという点で、ピコシュアプロがどう違うのかをお示ししました。
3. 講演でお話しした3つの違い
講演で対象として取り上げたのは、シミ・アザ（皮膚の色の異常）の治療です。効果の現れ方は、シミやアザの種類によっても異なります。
講演では、従来機との違いとして次の3点をお話ししました。
- 低い出力で、深い位置のメラニン（肌の色をつくる色素）を効率よく破壊する
- ダウンタイム（治療後に肌が落ち着くまでの期間）の軽減が期待できる（個人差があります）
- 少ない回数での治療が期待できる（個人差があります）

In 2022, at the 45th Annual Meeting of the Japan Society of Aesthetic Plastic Surgery (JSAPS2022) held in Sapporo, I gave a lecture on PicoSure Pro (PICOSURE pro), which I have been involved with since its development stage. The lecture centred on one of the points that determine the outcome of pigmentation treatment: “how deep into the skin the laser reaches”. I explained that, whereas with the conventional PicoSure the depth of penetration did not change even when the power was increased, PicoSure Pro can target melanin at deeper levels at lower power. In the same year, 2022, I also gave a lecture on PicoSure Pro at the Japanese Society of Aesthetic Dermatology meeting (JSAD2022).
Introduction
When treating pigmentation with a laser, there are important points that determine the outcome. At the 2022 Annual Meeting of the Japan Society of Aesthetic Plastic Surgery, I spoke about PicoSure Pro, focusing on one of them: “how deep into the skin the laser reaches”.
1. The 45th Annual Meeting of the Japan Society of Aesthetic Plastic Surgery in Sapporo
That year’s Annual Meeting of the Japan Society of Aesthetic Plastic Surgery (JSAPS2022) was the 45th, and it was held in Sapporo. The theme of my lecture was the picosecond laser “PicoSure Pro”.
PicoSure Pro is a picosecond laser that substantially improves on its predecessor, PicoSure. I was involved in its development as an Advisory board member of Cynosure, and at the time our clinic owned the first unit in the world.
2. Raising the power did not change how deep the laser reached
When the aim was to remove deep pigmentation thoroughly, with the conventional PicoSure the depth of penetration (how deep the laser reaches) did not change even when the power was increased.
The PicoSure we used at our clinic was also an excellent laser. In the lecture, I showed how PicoSure Pro differs in terms of this depth of reach.
3. The three differences I discussed in the lecture
The lecture focused on the treatment of pigmentation and birthmarks (abnormalities of skin colour). How results appear also differs depending on the type of pigmentation or birthmark.
In the lecture, I discussed the following three points as differences from the previous device.
- It efficiently breaks down melanin (the pigment that gives skin its colour) located at deeper levels, at lower power
- Reduced downtime (the period until the skin settles after treatment) can be expected (results vary between individuals)
- Treatment with fewer sessions can be expected (results vary between individuals)

The session was chaired by Dr. Taro Kono of the Department of Plastic Surgery, Tokai University. I would also like to thank once again Dr. Seiko Ko of KO CLINIC, with whom I shared the session.
In the same year, 2022, I also gave a lecture on PicoSure Pro at the Japanese Society of Aesthetic Dermatology meeting (JSAD2022), where I discussed the differences from PicoSure in terms of the depth of action and energy.

What kind of lecture did you give at JSAPS2022?
At the 45th Annual Meeting in Sapporo, I gave a lecture on the theme of PicoSure Pro. Why does the depth the laser reaches matter in pigmentation treatment?
In the lecture, I described the depth the laser reaches as one of the important points that determine the outcome of pigmentation treatment. On that basis, I took up the relationship between power and depth of reach as a limitation of the conventional PicoSure. Pigmentation and birthmarks differ in condition depending on their type, and the suitable treatment and how results appear also differ from person to person.
How does PicoSure Pro differ from PicoSure?
In the JSAPS2022 lecture, I focused mainly on the difference in depth of reach. With PicoSure Pro, it is possible to aim to efficiently break down melanin in deeper layers at lower power. At JSAD2022, in the same year, 2022, I explained the differences between the two in terms of the depth at which the laser acts and energy. Results vary between individuals.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even with the same settings. What creates that difference is whether the skin is in a state to receive heat and respond to treatment. That is why, while preparing the skin so that it can respond where necessary, I propose appropriate treatments in an appropriate order.
I placed this idea at the starting point of treatment design, systematised it as the “Thermal Thread Technique®”, and published it in “Lasers in Surgery and Medicine” in 2024. The paper has been selected as one of the journal’s Top Viewed and Top Cited Articles. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
I am a specialist of the American Society for Laser Medicine and Surgery (ASLMS) and Chair of the Board of Koyukai Medical Corporation. As the only participant from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have also reported cases treated with the first unit. I have given invited lectures at international congresses including ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also involved in teaching and training doctors overseas.
Even for those who have not achieved the results they hoped for with previous treatments, I assess why that symptom is occurring in that skin and propose treatments starting from the order in which they are needed.
