はじめに
シミを治療する「ピコレーザー」が日本に上陸してから、およそ8年。多くの企業からピコレーザーが発表され、機種はかなり増えていました。2022年の日本美容皮膚科学会（JSAD2022）で、私はその中のひとつ、ピコシュアプロについてお話ししました。
1. ピコシュアプロの登場と、第1号機の導入から約1年
ピコシュアプロは、Cynosure社のピコレーザー「ピコシュア（PicoSure）」の最新のハイエンドタイプとして登場しました。前身のピコシュアに大幅な改良を加えたレーザーで、私はCynosure社のAdvisory board memberとして開発段階から携わってきました。
当時、美容医療の機器の多くは海外で開発されていました。それをそのまま日本人の敏感な肌に使うことはできず、いかに日本人向けに調整していけるかがポイントだと、私は考えていました。
2021年秋、完成した第1号機が世界に先駆けて当院に導入されました。約半年の臨床試験期間を経て、2022年3月には東京・虎ノ門ヒルズで開かれたJeisys users meetingで機器を披露しました。
JSAD2022での講演は、導入からおよそ1年の時点で行ったものです。その間、ピコシュアプロを使って、シミ治療、ニキビ痕治療、小じわ治療などに取り組んできました。
2. 「今までのピコシュアと何が変わったのか？」への答え
ピコシュアプロについて、私が質問を受けることが多かったのが、この点です。講演でも、まずここを取り上げました。
少し専門的な話になりますが、大きな違いは「レーザーが作用する深さ」と「レーザーのエネルギー」です。ピコシュアプロは、ピコシュアに比べて、より深い位置まで、より強いレーザーエネルギーを作用させることができます。
従来のピコシュアでは、出力を上げても深達度（レーザーが届く深さ）は変わりませんでした。レーザーが皮膚のどこまで届くかは、シミ治療の結果を左右する大切なポイントのひとつです。
この違いによって、ピコシュアより少ない回数での治療が期待できます。ただし、実際に必要な回数には個人差があります。
3. シミの種類によって変わる照射の考え方
シミ治療は、シミの種類や深さ・厚さによって、進め方が変わってきます。講演では、シミの種類によって照射の方法が変わる考え方を説明しました。
シミの中には、ADM（別名：ABNOM、Hori’s nevus）のように、皮膚の深い位置にある種類もあります。ピコシュアプロの「深部まで強く届く」特徴は、こうした深さの違いを踏まえて照射を考えるときの要素のひとつでした。
どの照射が適しているかはシミの状態によって異なり、効果や経過の現れ方は人によって違います。
4. 2022年、国内外で重ねたピコシュアプロの講演
同じ2022年には、大阪で開かれた第65回日本形成外科学会総会で、通常であれば2名の講演が行われる50分の時間をいただき、レーザーの基礎から応用までをお話ししました。従来のピコレーザーとの違い、効果的な使用法、使用上の注意点まで扱った講演です。
日本美容外科学会（JSAS2022）では、最新機器の使い方を扱うセッションで座長兼演者を務めました。ポテンツァのアップデートについては今泉先生にお話しいただき、私はピコシュアプロを担当しました。札幌での第45回美容外科学会総会（JSAPS2022）でも、ピコシュアプロについて講演しました。
海外では、アメリカのASLMS2022でピコシュアプロとポテンツァを組み合わせた肌質改善治療を発表しました。オーストラリアのCOSDOC2022やタイ・バンコクのIMCAS Asia 2022では、この2つを組み合わせた治療「サイノグロー（CynoGlow）」について講演しました。
5. 同じ2022年の講演と、横浜あざみ野での診療
このセッションでは、近畿大学名誉教授の川田先生に座長を務めていただきました。改めて感謝申し上げます。

よくある質問
ピコシュアとピコシュアプロの違いは何ですか？
大きな違いは、レーザーが作用する深さと、レーザーのエネルギーです。ピコシュアプロは、ピコシュアに比べて、より深い位置まで、より強いエネルギーを作用させることができます。JSAD2022の講演でも、まずこの2点から説明しました。どのような照射が適しているかはシミの種類によって異なり、効果には個人差があります。
ADMとはどのようなシミですか？
ADMは、ABNOMやHori’s nevusとも呼ばれ、皮膚の深い位置にある種類のシミです。JSAD2022の講演では、シミのタイプに応じて照射を変える考え方を説明しました。深い位置にあるシミの治療の進め方や経過は、一人ひとり異なります。
ピコシュアプロは、どのような治療に使われてきましたか？

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

Key points
At the 2022 meeting of the Japanese Society of Aesthetic Dermatology (JSAD2022), I gave a lecture on PicoSure Pro (PICOSURE pro). Around a year after the first unit was installed at our clinic, I explained how it differs from PicoSure from two perspectives: the depth at which the laser acts and the laser energy. I also discussed the idea that the treatment method changes depending on the type of pigmentation. The session was chaired by Dr. Kawada, Professor Emeritus at Kindai University, and at the same meeting I also gave a lecture on Cyspera.
Introduction
Around eight years had passed since “picosecond lasers” for treating pigmentation arrived in Japan. Many companies had launched picosecond lasers, and the number of models had grown considerably. At the 2022 meeting of the Japanese Society of Aesthetic Dermatology (JSAD2022), I spoke about one of them, PicoSure Pro.
1. The arrival of PicoSure Pro, and about a year since the first unit was installed
PicoSure Pro was introduced as the latest high-end version of Cynosure’s picosecond laser, PicoSure. It is a laser with substantial improvements over its predecessor, PicoSure, and I have been involved with it since the development stage as a Cynosure Advisory board member.
At the time, most aesthetic medical devices were developed overseas. They could not simply be used as they were on the sensitive skin of Japanese patients, and I considered the key point to be how well they could be adjusted for Japanese patients.
In autumn 2021, the first completed unit was installed at our clinic, ahead of anywhere else in the world. After a clinical trial period of about six months, I presented the device at the Jeisys users meeting held at Toranomon Hills, Tokyo, in March 2022.
The JSAD2022 lecture was given about a year after the installation. During that time, I had been using PicoSure Pro for the treatment of pigmentation, acne scars, fine wrinkles and other concerns.
2. The answer to “What has changed from the previous PicoSure?”
This was the question I was most often asked about PicoSure Pro. In the lecture, too, I addressed this point first.
This becomes a little technical, but the main differences are “the depth at which the laser acts” and “the laser energy”. Compared with PicoSure, PicoSure Pro can deliver stronger laser energy to a deeper level.
With the previous PicoSure, raising the output did not change the depth of penetration (the depth the laser reaches). How far the laser reaches into the skin is one of the important factors that determine the outcome of pigmentation treatment.
Because of this difference, treatment with fewer sessions than with PicoSure can be expected. However, the number of sessions actually needed varies between individuals.
3. How the approach to treatment changes with the type of pigmentation
How pigmentation treatment proceeds depends on the type, depth and thickness of the pigmentation. In the lecture, I explained the idea that the treatment method changes with the type of pigmentation.
Some types of pigmentation, such as ADM (also known as ABNOM or Hori’s nevus), lie deep in the skin. PicoSure Pro’s characteristic of “reaching deep with strength” was one of the factors to consider when planning treatment in light of these differences in depth.
Which treatment is suitable depends on the condition of the pigmentation, and how results and the course of treatment appear differs from person to person.
4. 2022: a series of PicoSure Pro lectures in Japan and abroad
In the same year, 2022, at the 65th Annual Meeting of the Japan Society of Plastic and Reconstructive Surgery in Osaka, I was given a 50-minute slot that would normally accommodate two lectures, and spoke about the laser from the basics to its applications. The lecture covered the differences from conventional picosecond lasers, effective methods of use and precautions in use.
At the Japan Society of Aesthetic Surgery meeting (JSAS2022), I served as both chair and speaker in a session on how to use the latest devices. Dr. Imaizumi spoke about the Potenza update, and I covered PicoSure Pro. I also lectured on PicoSure Pro at the 45th Congress of the Japan Society of Aesthetic Plastic Surgery (JSAPS2022) in Sapporo.
Abroad, I presented a skin-quality improvement treatment combining PicoSure Pro and Potenza at ASLMS2022 in the United States. At COSDOC2022 in Australia and IMCAS Asia 2022 in Bangkok, Thailand, I lectured on “CynoGlow”, a treatment that combines the two.
5. Another lecture in 2022, and our practice in Azamino, Yokohama
This session was chaired by Dr. Kawada, Professor Emeritus at Kindai University. I would like to thank him once again.
At the same JSAD2022, I also gave a lecture on the new line-up of the skin-brightening cream “Cyspera”.
In 2022, at our clinic in Azamino, Yokohama, we used a method of applying special laser treatment tailored to the type of pigmentation across the whole face.

What is the difference between PicoSure and PicoSure Pro?
The main differences are the depth at which the laser acts and the laser energy. Compared with PicoSure, PicoSure Pro can deliver stronger energy to a deeper level. In the JSAD2022 lecture, I began by explaining these two points. Which treatment is suitable depends on the type of pigmentation, and results vary between individuals.
What kind of pigmentation is ADM?
ADM, also called ABNOM or Hori’s nevus, is a type of pigmentation that lies deep in the skin. In the JSAD2022 lecture, I explained the idea of changing the treatment according to the type of pigmentation. How treatment of deep pigmentation proceeds, and its course, differ from person to person.
What treatments has PicoSure Pro been used for?
In the roughly one year after the first unit was installed at our clinic, I used it for the treatment of pigmentation, acne scars, fine wrinkles and other concerns. In the same year, 2022, I also presented a skin-quality improvement treatment combining it with Potenza at society meetings in Japan and abroad. Suitability and the course of treatment differ from person to person.

When addressing concerns that appear on the skin, such as pigmentation, melasma or pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when the same settings are used. What creates that difference is whether the skin is in a state to receive the heat and respond to the treatment. The paper has been selected as a Top Viewed and Top Cited Article in that journal. The radiofrequency treatment technique “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, have also been published in the “Journal of Cosmetic Dermatology”.

As the only participant from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since the development stage and have also reported cases treated with the first unit. I have given invited lectures at international congresses such as ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”, also working on the teaching and training of physicians overseas.
Even for those who have not seen the response they hoped for from previous treatments, I assess why those symptoms are occurring in that skin and propose treatment starting from the order in which it is needed.
