- 講演②:PICOSUREpro(ピコシュアプロ)の臨床活用術 ― 8年間の経験から

Director Kentaro Oku | Record of a lecture given on 31 August 2024 (expanded and restructured on 20 October 2026)
Key points
In 2024, at the meeting of the Japanese Society of Aesthetic Dermatology held at the Nagoya Congress Center, I gave lectures on three themes. On Sofwave and the Thermal Thread Technique® (thermal thread method), I spoke about a method that aims for both a “diagonal upward lift” and skin-quality improvement at the same time; on PICOSUREpro (PicoSure Pro), drawing on eight years of experience, I spoke about the basic principle of “distance” during treatment; and on Wincell (Windscell), I spoke about how “AIR”, water particles of about 1.4 nm, works. The content offers a starting point for understanding that the safety of laser treatment depends not only on the device but also on the basic technique of the treatment itself.

Introduction
“I performed the treatment using the method published in a paper, but the side reactions were strong. Could this be a fault in the device?” In my PICOSUREpro lecture, I answered questions like this one, which I am often asked. Here I summarise the three lectures I gave at the 2024 meeting of the Japanese Society of Aesthetic Dermatology, in a form that patients can also read.
1. Three lecture themes in Nagoya, with a typhoon approaching
The 2024 meeting of the Japanese Society of Aesthetic Dermatology was held at the Nagoya Congress Center. With a typhoon approaching, whether we could “even reach” the venue became a major question at this congress. Because the shinkansen was suspended, I had to travel to Nagoya a day early at short notice. I am sincerely grateful to everyone who kindly accommodated the change of plans.
At this society meeting, one of the major congresses in the field of aesthetic dermatology in Japan, I gave lectures on the following three themes.
- Lecture 1: Sofwave™ and the Thermal Thread Technique® (thermal thread method) in practice, with a live demonstration
- Lecture 2: Clinical use of PICOSUREpro (PicoSure Pro) — from eight years of experience
- Lecture 3: The technology and mechanism of Wincell (Windscell), a device developed in Japan

2. Sofwave and the Thermal Thread Technique® — why the lift goes “diagonally upwards”
The Thermal Thread Technique® (hereafter TTT) is a technique that aims for both a “diagonal upward lift” and skin-quality improvement at the same time using ultrasound. In this lecture, I explained, through the talk and a live demonstration, a method of addressing wrinkles and sagging that focuses on the Relaxed Skin Tension Lines (RSTL: lines showing the directions in which the skin tends to wrinkle).
Since I published TTT in a paper, Sofwave has become the device I am most often invited to lecture on. Sofwave’s distinctive feature is its use of “activating” ultrasound. The feature of TTT is that, when TTT is used with this device, it can aim for both a “diagonal upward lift” and skin-quality improvement at the same time. The degree of change varies between individuals.
Why does the skin lift “diagonally upwards”, as it does when you tie your hair in a ponytail? This is the key point of TTT. Our skin keeps sustaining damage over a long period, but the “direction” in which wrinkles and sagging form follows a general pattern. These are the RSTL. It is similar to the way a crease forms when you fold paper in the same place every time.
If the thermal energy from the ultrasound can be “linked” so as to counteract these wrinkles, an effect like being lifted diagonally upwards can be expected as a result. It may sound simple, but when it comes to actually performing the treatment, a number of points have to be cleared, so this time I focused my talk on those points.

3. PICOSUREpro — the basic principle of treatment “distance”, from eight years of experience
Our clinic introduced the first PICOSUREpro unit in 2021. Before that, we had been using its predecessor, PicoSure, since 2016, giving us eight years of experience in total.
Through training at GMI (Global Medical Institute: our clinic’s educational institution) and lectures at overseas congresses, I have received many questions, and many of them shared common concerns. This time, I held a session answering those questions in a form that can be put to use in clinical practice straight away. The chair was Professor Kono of the Department of Plastic Surgery, Tokai University, a mentor I deeply respect.
As for the question at the beginning, the possibility of a device fault is not zero, but it can be considered very low. Medical devices are highly stable, and the stability of PICOSUREpro is also ensured through regular maintenance. Keeping the external environment, such as temperature and humidity, in order is also important for maintaining high stability.
More important than that is the “distance” during treatment. How accurately the laser energy is delivered to the skin is determined by the distance between the handpiece (the hand-held part that emits the laser) and the skin surface. If the handpiece is even slightly away from the skin, the energy delivered may be amplified to around twice what the practitioner intended, and this causes unexpected side reactions. I explained that understanding these basics and managing them carefully is important for preventing problems.
4. Wincell (Windscell), a device developed in Japan, and “AIR”, water particles of about 1.4 nm
The third lecture started from the basics: “What is Wincell in the first place?” Wincell is a device developed in Japan, and I believe it holds great potential.
What this device uses is “water”. However, it is not ordinary water but water particles that have been turned, by a special technology, into clusters (groups of particles) 1.4 nm (nanometres: one millionth of a millimetre) in size; these are called AIR. This AIR is released from Wincell and acts on the skin.
It is entirely different from the water we encounter every day; to use an analogy, it follows the same idea as a “laser”. Just as a laser amplifies a specific part of light and uses it to improve pigmentation and rejuvenate the skin, AIR is made by extracting and gathering water particles of a specific size, and it has its own distinctive action.
At our clinic, we use this AIR after laser treatment to help reduce downtime (the period until recovery). How quickly the skin recovers varies between individuals. In the lecture, I described the following three actions of AIR.
- Soothing (calming the skin)
- Moisturising
- Drug delivery (delivering active ingredients to the skin)
5. Exchanges with doctors visiting from overseas
Many doctors active overseas came to Japan for the 2024 meeting of the Japanese Society of Aesthetic Dermatology, including Dr. Rox Anderson, a leading figure in laser medicine. I often meet them at overseas congresses, but the chance to talk with them at length in Japan was extremely valuable, and it was a very meaningful time.
At this congress, I was able not only to share technical knowledge but also to gain new stimulus and learning. I will put these experiences to use in my daily practice in Azamino, Yokohama.
Frequently asked questions
What is the Thermal Thread Technique® (TTT) with Sofwave?
It is a treatment technique that aims for both a “diagonal upward lift” and skin-quality improvement at the same time by “linking” the thermal energy from Sofwave’s ultrasound. It focuses on the RSTL (Relaxed Skin Tension Lines), the directions in which wrinkles and sagging form, and links the energy so as to counteract those wrinkles. There are a number of points to clear in performing the treatment, and at the Japanese Society of Aesthetic Dermatology 2024 I focused my talk on them. How the treatment feels and the degree of change vary between individuals.
Why is the distance between the handpiece and the skin important?
Because how accurately the laser energy is delivered to the skin is determined by the distance between the handpiece and the skin surface. If the handpiece is even slightly away from the skin, the energy delivered may be amplified to around twice what was intended, which causes strong side reactions. The possibility of a device fault is very low, and the stability of PICOSUREpro is also ensured through regular maintenance. Managing these basics carefully is important for preventing problems.
What kind of device is Wincell (Windscell)?
It is a device developed in Japan that releases “AIR”, water particles turned by a special technology into clusters about 1.4 nm in size. In the lecture, I described three actions of AIR: soothing, moisturising and drug delivery. At our clinic, we use it with the aim of reducing downtime after laser treatment. How quickly the skin recovers varies between individuals.
