はじめに
ステージに立っているのが誰なのか分からないほど広い会場が、ありがたいことに満員になりました。2019年8月、マレーシアのクアラルンプールで開かれた国際美容皮膚科学会ADAC2019での講演です。さまざまな分野のエキスパートが講演するとあって、アジア各国からドクターが集まっていました。
1. クアラルンプールに集まったアジア各国の医師
ADAC2019のモデレーター（進行役）は、アジア太平洋地域で活躍されているDr.Woraphong、Dr.Rungsima、Dr.Tingsongのお三方です。会場は、クアラルンプールにあるThe Majestic Hotel Kuala Lumpur。モダンなコロニアル調のホテルでした。
当時のあざみ野ヒルズスキンクリニック（現 ヒルズグレイスクリニック）の院長として、ADAC2019で私に与えられたミッションは次の2つでした。
- ピコ秒（1兆分の1秒）レーザー「ピコシュア」の新たな使用方法
- 高周波（RF）機器「テンプシュア」の導入後の経過
2. 導入から数か月 ― テンプシュアの経過を報告
テンプシュアは高周波（RF）の熱を使う機器で、ダウンタイム（回復までの期間）を抑えながら、タイトニング（皮膚の引き締め）とリフトアップを狙うものです。当時、当院はアジアで初めてテンプシュアを導入した施設でした。
ADAC2019の時点で、導入から数か月がたっていました。講演では、初期の経過と、その後の中期的な経過を、実際の症例とともにお話ししました。
テンプシュアは熱の入れ方を工夫することで、施術直後に感じる変化と、少し時間がたってから現れる変化の両方が期待できます。感じ方や変化の程度には個人差があります。
3. ピコシュアで重症ニキビに向き合う
日本を含むアジアでは、ピコシュアはシミ治療に使われることの多い機器です。一方で、ピコシュアによるニキビ治療は、ASLMS（アメリカレーザー医学会）で講演したあとに大きな反響をいただきました。ニキビで悩む方への新たな道を切り拓けるよう、当時の私はアジア各国で講演を重ねていました。
その前段となったASLMSでは、活動性ニキビ（今まさに炎症を起こしているニキビ）に対するピコシュア治療の有効性と安全性について口頭発表しました。当院では、ピコシュアの導入後まもなくニキビ治療用の設定と照射方法を考え、治療に取り入れていました。
ニキビに対するピコシュアの照射法は、複雑かつ緻密です。ADAC2019では、当時、世界的な報告がまだ見当たらなかった、ピコシュアによる重症ニキビ治療について詳しく説明しました。どの程度の変化が見込めるかは、ニキビの状態や肌質によって異なり、個人差があります。
4. 他のセッションでの学びと、横浜あざみ野の診療
自分の講演以外にも他のセッションに参加し、ドクターごとの考え方や、国による違い、治療の組み立て方の工夫など、多くのことを学ばせていただきました。
会場では、シンガポールのDr.Kenneth、台湾のDr.Peter、ベトナムのDr.Huy、そしてCynosureのMr.Michel Byersともご一緒する機会がありました。国際学会ではいつも、各国の優秀なドクターたちとアイデアをシェアすることができ、新たな方向性が見えてきます。
学会を終えて、私はこの学びを早速診療に生かしていこうと考えていました。各国の先生方との議論で得た視点を、横浜あざみ野での日々の診療に生かしています。
よくある質問
ピコシュアはシミだけでなく、ニキビにも使えるのですか？
日本を含むアジアではシミ治療に使われることの多い機器ですが、照射法を工夫してニキビ治療に用いる方法もあります。ニキビに対する照射法は複雑かつ緻密で、肌の状態に合わせた設定が必要です。効果の出方には個人差があり、すべての方に向くわけではありません。診察でニキビと肌の状態を確認したうえで、治療をご提案します。
テンプシュアはどのような機器ですか？

海外の学会での学びは、日本の診療にどう生かされるのですか？
国際学会では、各国の医師の考え方や治療の組み立て方の違いを知ることができます。そのまま取り入れるのではなく、日本で治療を行う際に気をつけるべき点を考えたうえで、診療に反映させています。どの治療が合うかは肌の状態によって異なり、効果には個人差があります。

ADAC2019は、ピコシュアとテンプシュアという2つの機器について、アジア各国の先生方と考えを共有する場になりました。ベトナムの先生方へのピコシュア講義や、ニキビ治療の反響が始まったASLMSでの講演も、同じシリーズの記事で振り返っています。

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

Key points
In August 2019 I gave a lecture at ADAC2019 (Aesthetic Dermatology Academy Conference), an international aesthetic dermatology conference held in Kuala Lumpur, Malaysia. The topics were a treatment technique for severe acne as a new use of the picosecond laser PicoSure, and a report on the course of treatment with the radiofrequency (RF) device TempSure Envi, which our clinic had introduced only a few months earlier. In other sessions, I learned about differences between countries in ways of thinking and in how treatments are planned. I apply what I learned to how I plan treatments in Azamino, Yokohama.
Introduction
The venue was so large that you could hardly tell who was standing on the stage, and I am grateful that it was full. This was my lecture at ADAC2019, the international aesthetic dermatology conference held in Kuala Lumpur, Malaysia, in August 2019. With experts from a wide range of fields giving lectures, doctors had gathered from across Asia.
1. Doctors from across Asia gathered in Kuala Lumpur
The moderators of ADAC2019 were three doctors active in the Asia-Pacific region: Dr. Woraphong, Dr. Rungsima and Dr. Tingsong. The venue was The Majestic Hotel Kuala Lumpur, a modern colonial-style hotel.
As Director of what was then Azamino Hills Skin Clinic (now Hills Grace Clinic), I was given the following two missions at ADAC2019:
- A new way of using the picosecond (one-trillionth of a second) laser PicoSure
- The course of treatment after introducing the radiofrequency (RF) device TempSure Envi
2. A few months after introduction: reporting on the course of TempSure Envi treatment
TempSure Envi is a device that uses radiofrequency (RF) heat to aim for tightening (firming of the skin) and lifting while keeping downtime (the time needed to recover) low. At the time, our clinic was the first facility in Asia to introduce TempSure Envi.
By the time of ADAC2019, a few months had passed since we introduced TempSure Envi. In the lecture, I presented the early course and the subsequent medium-term course, together with actual cases.
By adjusting how heat is delivered, TempSure Envi can be expected to produce both changes felt immediately after treatment and changes that appear after some time has passed. How it feels and the degree of change vary between individuals.
3. Addressing severe acne with PicoSure
In Asia, including Japan, PicoSure is a device often used to treat pigmentation. On the other hand, acne treatment with PicoSure drew a great response after my lecture at ASLMS (American Society for Laser Medicine and Surgery). At the time, I was giving lecture after lecture across Asia in the hope of opening up a new path for people troubled by acne.
At ASLMS, which preceded this, I gave an oral presentation on the efficacy and safety of PicoSure treatment for active acne (acne that is currently inflamed). Soon after introducing PicoSure, our clinic devised settings and a treatment technique for acne and incorporated them into treatment.
The PicoSure treatment technique for acne is complex and meticulous. At ADAC2019, I explained in detail the treatment of severe acne with PicoSure, for which no reports could yet be found anywhere in the world at the time. The degree of change that can be expected differs depending on the condition of the acne and the skin type, and results vary between individuals.
4. Lessons from other sessions, and practice in Azamino, Yokohama
Besides my own lecture, I attended other sessions and learned a great deal, including each doctor’s way of thinking, differences between countries and ingenuity in how treatments are planned.
At the venue, I also had the opportunity to spend time with Dr. Kenneth from Singapore, Dr. Peter from Taiwan, Dr. Huy from Vietnam and Mr. Michel Byers of Cynosure. International conferences always let me share ideas with excellent doctors from many countries, and new directions come into view.
After the conference, I was intent on putting what I had learned into practice straight away. I apply the perspectives gained from discussions with doctors from various countries to my daily practice in Azamino, Yokohama.

Can PicoSure be used for acne as well as pigmentation?
In Asia, including Japan, PicoSure is a device often used to treat pigmentation, but there is also a way of using it for acne treatment by adapting the treatment technique. The treatment technique for acne is complex and meticulous, and the settings must be matched to the condition of the skin. How results appear varies between individuals, and it is not suitable for everyone. We propose treatment after checking the acne and the condition of the skin at an examination.
What kind of device is TempSure Envi?
TempSure Envi is a device that uses radiofrequency (RF) heat to aim for skin tightening (firming) and lifting while keeping downtime low. How is what you learn at overseas conferences applied to practice in Japan?
At international conferences, I can learn about differences in how doctors from various countries think and plan treatments. Rather than adopting these approaches as they are, I reflect them in my practice after considering the points that need care when providing treatment in Japan. Which treatment suits you depends on the condition of your skin, and results vary between individuals.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when treatment is given at the same settings. What creates that difference is whether the skin is in a state to receive heat and respond to treatment. That is why, where necessary, I prepare the skin so that it can respond, and propose the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper was selected as a Top Viewed and Top Cited Article in the journal. I have also published the radiofrequency treatment technique “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
ASLMS specialist (American Society for Laser Medicine and Surgery) and Chair of the Board of Koyukai Medical Corporation. As the only member from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have reported cases using the first unit. I have given invited lectures at international conferences including ASLMS, the world’s largest laser medicine society, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also engaged in teaching and training doctors overseas.
Even for those who have not achieved the results they hoped for with previous treatments, I assess why those symptoms are occurring in their skin and propose treatment starting from the order in which it is needed.
