はじめに
「肌質改善」や「肌育」は、日本だけでなく世界的にも注目されているテーマです。2024年、台湾・高雄で開かれたISDS（国際皮膚美容外科学会）で、このテーマをいただいて講演する機会がありました。ここでは、講演でお話しした治療の組み立てと、学会で得た学びをまとめます。
1. 台湾・高雄のISDSで「肌質改善」をテーマに講演
会場は台湾・高雄。ISDS（国際皮膚美容外科学会）の大会会頭であるDr. Peter Pengからご依頼をいただき、「肌質改善」をテーマに講演させていただきました。
その中で私がお話ししたのは、肌の「透明感」を軸にしたアプローチです。その中核を担うのが、ピコシュアプロとボライト注入を組み合わせるコンビネーション治療でした。
2. 一つの治療だけでは届きにくい ― コンビネーション治療という考え方
講演でまずお伝えしたのは、肌質改善は一つの治療だけで達成するのが難しい、という点です。ピコシュアプロはレーザーによる治療、ボライトはヒアルロン酸を肌に注入する治療で、それぞれに異なる特長があります。その特長を組み合わせることで、単独の治療では届きにくい部分を補い合うことを狙います。
2つを組み合わせて目指すのは、「明るさ」「ハリ」「透明感」を兼ね備えた肌です。ただし、どの程度の変化が得られるか、どのくらい続くかには個人差があります。組み合わせ方や順番は、お肌の状態を診察したうえで判断します。
3. ピコシュアプロの役割 ― メラニン・ターンオーバー・質感
講演では、肌質改善の中でピコシュアプロが担う役割を、次の3つに整理してお話ししました。
- メラニン（肌の色をつくる色素）への作用：特定の波長でメラニンをターゲットにし、シミやくすみの改善が期待できます。
- ターンオーバー（肌が生まれ変わるサイクル）の促進：古い角質を除去し、新しい肌細胞が生まれるのを促します。
- フラクショナルモード（細かな点状に照射する方法）：微細な刺激がコラーゲン（肌のハリを支えるたんぱく質）の生成を促し、肌全体の質感の向上が期待できます。
4. ボライトの役割 ― 保水・ハリ・トーン
もう一方のボライトは、ヒアルロン酸の注入治療です。レーザーとは働きかける場所が異なり、肌の内側からの潤いやハリを担います。講演では、ボライトの役割も次の3つに分けて説明しました。
- 保水：肌の内部から潤いを補い、乾燥による小ジワの改善が期待できます。
- ハリの回復：ヒアルロン酸の働きで、肌に弾力を与えることを目指します。
- トーンアップ：肌のトーンを均一に整え、明るく透明感のある肌を目指します。
5. FACE-Qという評価方法と、食事の席で得た学び
講演では、2つの治療を組み合わせるときの考え方を解説しました。あわせて、FACE-Q（患者様ご自身の回答をもとに治療結果を評価する指標）という評価方法についても紹介しました。医師の見立てだけでなく、治療を受けた方の側から結果を見るための指標です。
各国の医師にも興味を示していただき、透明感を重視したアプローチが美容医療のトレンドとして注目されていることを実感しました。
ISDSでの学びは、講演の場だけではありませんでした。食事を囲みながらのカジュアルな時間に、「今何が一番注目されているか」「次に取り組むべきテーマは何か」といった話題が飛び交います。こうしたリラックスした交流の中で得る発見は、学会ならではの魅力の一つです。
国際的な場で共有した、透明感を中心に据えた肌質改善の考え方は、横浜あざみ野での日々の診療でも、今後さらに追求していきたいテーマです。

肌質改善は、一つの治療だけでは難しいのですか？

ピコシュアプロとボライトは、それぞれ何に働きかけるのですか？
ピコシュアプロは、特定の波長でメラニン（肌の色をつくる色素）をターゲットにしてシミやくすみに働きかけること、ターンオーバーを促すこと、フラクショナルモードの微細な刺激でコラーゲンの生成を促すことの3つを担います。ボライトは、保水、ハリの回復、肌トーンを整えるという3つの役割です。2つを組み合わせて「明るさ」「ハリ」「透明感」を目指しますが、効果の出方には個人差があります。
FACE-Qとは何ですか？
FACE-Qは、患者様ご自身の回答をもとに治療結果を評価する指標です。満足度を多角的に測る仕組みで、医師の見立てとは別に、治療を受けた方の側から結果を把握することを目的としています。ISDSの講演では、この指標についてもご紹介しました。治療結果の感じ方には個人差があります。

米国レーザー医学会（ASLMS）専門医、医療法人社団光悠会理事長。Cynosure社のアドバイザリーボードには日本から唯一参加し、PicoSure Proの開発段階から携わり、第1号機による症例報告も行ってきました。世界最大のレーザー医学会ASLMSやIMCAS ASIA、KOREA DERMAなどの国際学会で招待講演を行い、国際的な医師教育プログラム「GMI」を主宰。海外の医師の指導・育成にも取り組んでいます。

Key points
In 2024, at the ISDS (International Society for Dermatologic and Aesthetic Surgery) congress held in Kaohsiung, Taiwan, I gave a lecture on the theme of “skin-quality improvement” at the request of the congress president, Dr. Peter Peng. I spoke about an approach focused on the “translucency” of the skin, at the core of which I placed a combination treatment of PicoSure Pro and Volite (now SKINVIVE) injections. The idea is to combine two treatments that work differently, a laser and a treatment that injects hyaluronic acid into the skin, to aim for “brightness”, “firmness” and “translucency” together; results vary between individuals. In the lecture I explained the mechanism of the combination, and physicians from various countries showed interest.
Introduction
“Skin-quality improvement” and “skin cultivation” are themes attracting attention not only in Japan but worldwide. In 2024, I had the opportunity to give a lecture on this theme at the ISDS (International Society for Dermatologic and Aesthetic Surgery) congress held in Kaohsiung, Taiwan. Here I summarise the treatment structure I presented in the lecture and what I learned at the congress.
1. A lecture on “skin-quality improvement” at ISDS in Kaohsiung, Taiwan
The venue was Kaohsiung, Taiwan. At the request of Dr. Peter Peng, president of the ISDS (International Society for Dermatologic and Aesthetic Surgery) congress, I gave a lecture on the theme of “skin-quality improvement”.
In it, I spoke about an approach centred on the “translucency” of the skin. At its core was a combination treatment pairing PicoSure Pro with Volite injections.
2. Hard to achieve with one treatment alone: the concept of combination treatment
The first point I made in the lecture was that skin-quality improvement is difficult to achieve with a single treatment alone. PicoSure Pro is a laser treatment and Volite is a treatment that injects hyaluronic acid into the skin, and each has different characteristics. By combining these characteristics, the aim is for each to complement what the other, used alone, finds hard to reach.
What the combination of the two aims for is skin that has “brightness”, “firmness” and “translucency” together. However, how much change is achieved and how long it lasts vary between individuals. How the treatments are combined and in what order is decided after examining the condition of your skin.
3. The role of PicoSure Pro: melanin, turnover and texture
In the lecture, I organised the role PicoSure Pro plays in skin-quality improvement into the following three points.
- Action on melanin (the pigment that gives skin its colour): PicoSure Pro targets melanin with a specific wavelength, and improvement of pigmentation and dullness can be expected.
- Promotion of turnover (the cycle in which the skin renews itself): PicoSure Pro removes old keratin and encourages the formation of new skin cells.
- Fractional mode (a method of treating in fine dot-like points): the fine stimulation encourages the production of collagen (the protein that supports the skin’s firmness), and an improvement in the texture of the skin as a whole can be expected.
4. The role of Volite: hydration, firmness and tone
The other treatment, Volite, is a hyaluronic acid injection. It acts on a different part of the skin from the laser and is responsible for moisture and firmness from within the skin. In the lecture, I also explained the role of Volite in the following three points.
- Hydration: Volite replenishes moisture from within the skin, and improvement of fine lines caused by dryness can be expected.
- Restoring firmness: through the action of hyaluronic acid, Volite aims to give the skin elasticity.
- Tone-up: Volite aims to even out skin tone for bright, translucent skin.
5. The FACE-Q assessment method, and lessons learned over meals
In the lecture, I explained the thinking behind combining the two treatments. I also introduced an assessment method called FACE-Q (a measure that evaluates treatment results based on patients’ own responses). It is a measure for viewing results from the perspective of the person who received the treatment, not only from the physician’s assessment.
Physicians from various countries also showed interest, and I felt that an approach emphasising translucency is attracting attention as a trend in aesthetic medicine.
What I learned at ISDS was not limited to the lecture itself. During casual time over meals, topics such as “what is attracting the most attention right now” and “what theme should we tackle next” came up one after another. The discoveries made in such relaxed exchanges are one of the particular attractions of a congress.
The approach to skin-quality improvement centred on translucency that I shared on the international stage is a theme I want to pursue further in my daily practice in Azamino, Yokohama.
Frequently asked questions
Is skin-quality improvement difficult with just one treatment?
In my ISDS lecture, I said that skin-quality improvement is difficult to achieve with a single treatment alone. On that basis, I introduced the idea of combining treatments that act on different targets: the PicoSure Pro laser and Volite, which injects hyaluronic acid. Which treatment suits you, and whether to combine them, depends on the condition of your skin. Results vary between individuals, so please discuss this at a consultation.
What do PicoSure Pro and Volite each act on?
PicoSure Pro has three roles: targeting melanin (the pigment that gives skin its colour) with a specific wavelength to act on pigmentation and dullness, promoting turnover, and encouraging collagen production through the fine stimulation of fractional mode. Volite has three roles: hydration, restoring firmness and evening out skin tone. The combination of the two aims for “brightness”, “firmness” and “translucency”, but how the effects appear varies between individuals.
What is FACE-Q?
FACE-Q is a measure that evaluates treatment results based on patients’ own responses. It is a system for measuring satisfaction from multiple angles, and its purpose is to capture results from the perspective of the person who received the treatment, separately from the physician’s assessment. I also introduced this measure in my ISDS lecture. How treatment results are perceived varies between individuals.
In closing
The lecture at ISDS not only allowed me to share my approach to skin-quality improvement on an international stage, but was also a valuable opportunity to gain new perspectives and inspiration. I intend to put what I gained there to use in my daily practice in Azamino, Yokohama.

When addressing concerns that appear on the skin, such as pigmentation, melasma and pores, what I value is assessing not only the symptoms but also the condition of the skin in which those symptoms arose.
With devices such as lasers, radiofrequency and ultrasound, results differ from person to person even when treating with the same settings. What creates that difference is whether the skin is in a state where it can receive heat and respond to treatment. That is why, where necessary, I prepare the skin so that it can respond, while proposing the appropriate treatments in the appropriate order.
Placing this idea at the starting point of treatment design, I systematised it as the “Thermal Thread Technique®” and published it in “Lasers in Surgery and Medicine” in 2024. The paper has been selected as a Top Viewed and Top Cited Article of the journal. I have also published the radiofrequency treatment method “Mono-Bi CrossLIFT Technique” and the “CrossTALK® protocol”, which combines injectable treatments with medical devices, in the “Journal of Cosmetic Dermatology”.
Specialist of the American Society for Laser Medicine and Surgery (ASLMS) and Chair of the Board of Koyukai Medical Corporation. As the only member from Japan on Cynosure’s advisory board, I have been involved with PicoSure Pro since its development stage and have reported cases using the first unit. I have given invited lectures at international congresses including ASLMS, the world’s largest society of laser medicine, IMCAS ASIA and KOREA DERMA, and I lead the international physician education programme “GMI”. I am also involved in teaching and training physicians overseas.
Even for those who have not obtained the results they hoped for from previous treatments, I assess why those symptoms are occurring in that skin and propose treatment starting from the order in which it is needed.
